AUTOLOGOUS SERUM IN SURGERY FOR IDIOPATHIC MACULAR HOLE

Abstract

 Background & Purpose: To evaluate prospectively the risk and benefits of AUTOLOGOUS SERUM (AS) in surgery for idiopathic MACULAR HOLE.Design: Double masked, clinical trial on 43 patients.Interventions: Standardized MACULAR HOLE surgery versus surgery combined with injection of an AUTOLOGOUS SERUM (AS). In all 43 cases, the procedures consisted of three port pars plana VITRECTOMY, posterior hyaloid separation, peeling of visible epiretinal membrane when present and nonexpansile gas-fluid exchange. After the gas-fluid exchange, patients were randomized to receive either injection of an AS (serum group=25) or no adjunctive treatment (control group=18). After surgery, patients were positioned face down for at least 7 days.Results: One month after surgery the anatomic success rate in the serum group was 25 of 25 (100%) versus 16 of 18 (88.9%) in the control group (P= 0.16, fisher's exact test). Visual acuity improvement was not significantly different between the two groups. (P=0.45, fisher's exact test) There were no complications specifically attributable to the serum injection.Improvement of two or more lines of visual acuity was significantly associated with successful anatomical closure in both groups (P<0.05).Conclusion: Injection of AS did not significantly improve the anatomic success rate of surgery for idiopathic MACULAR HOLEs and post operative visual acuity of serum group was not statistically different from the control group.
