EFFECT OF TILTING THE ABLATION SURFACE ON ACHIEVED SPHERE AND INDUCED ASTIGMATISM IN EXCIMER SURGERY

Abstract

 Purpose: To describe a mathematical relationship between tiling the ABLATION SURFACE and achieved (in comparison to attempted) sphere and induced astigmatism during Excimer refractive surgery.Setting: Two excimer laser surgery centers, Tehran, Iran.Methods: In an experimental study attempted correction spheres of-3.0, -5.0, -7.0 and -10.0 diopters of ablation were performed over PMMA test plates. The Excimer machine used was Nidek EC5000, keeping the laser suite condition stable all along the experiment. The surface of the plate was ablated in complete horizontal (zero degree tilt) and later 10, 20 and 30 degrees tilt. The amount of correction in each tilted position of ablation was repeated ten times and then measured by a LM-50 Nidek lensometer. Irregularity of induced astigmatism was graded according to regularity of image in lensometry from grade 0 to 3. Mean and standard deviation for spheres achieved and induced astigmatism in each condition were measured and compared to attempted correction. Statistical analysis of data was performed with paired t-test and Chi-square.Results: Except correction of -3.0 diopters with 10 degrees tilt (P<0.1), all other tilted conditions resulted in statistically significant undercorrection (P<0.05). Increase in tilting angle was associated with increase in the amount of undercorrection. Except in horizontal (zero degree) position, irregularity of induced astigmatism was increased with increasing the tilting angle.Conclusion: To achieve predictable results in EXCIMER SURGERY ABLATION SURFACE must be completely horizontal (zero degree tilt) and the laser beam perpendicular to it. Predictability of surgery decreases as the tilt of ABLATION SURFACE is increased.
