BANDAGE CONTACT LENS AND TOPICAL INDOMETHACIN IN TREATING PERSISTANT EPITHELIAL DEFECT AFTER VITREO-RETINAL SURGERIES

Abstract

 Purpose: Persistant corneal abrasion is one of the presenting complaints after longstanding VITREO-RETINAL SURGERIES. The purpose of this study was to assess the efficacy of using BANDAGE CONTACT LENSes in conjunction with a topical INDOMETHACIN to achieve the goals of safety, the facilitation of wound healing and pain control.Methods: Between October 1995 and November 1999, patients presenting with persistant CORNEAL EROSION, were considered for the study. Patients were assigned randomly to one of two treatment groups. Patients in both groups received a therapeutic contact lens and chloramphenicol eye drop. Group A received INDOMETHACIN and group B, placebo eye drop. This was done in a single-masked randomized fashion. Patients were reviewed daily to monitor subjective symptoms, and to measure the maximum and minimum dimensions of the abrasions.Results: A total of 16 patients were analyzed. Subjective evaluation of follow-up day 1 of photophobia, foreign body sensation, headache, and tearing did not differ significantly between the two groups. The difference in the mean pain decrease between group A and B on follow-up day one was significant. In group A, complete healing of the erosion occurred in 25% by day 4, 37.5% by day 5, and 37.5% by day 6. Pain reduction on the first follow-up day was reported -3 (maximum possible decrease) in no patient, -2 in 25%, -1 in 25% and no change in 50%. In group B, healing occurred in 50% by day 4,25% by day 5, and 25% by day 7. Pain reduction on the first follow-up day was reported -3 (maximum possible decrease) in 12.5%, -2 in 62.5%, and -1 in 25%.Conclusion: The study illustrated the effectiveness of INDOMETHACIN ophthalmic drop in providing improved discomfort in treating persistant corneal abrasion after VITREO-RETINAL SURGERIES.
