Abstract

Intracytoplasmic sperm injection (ICSI) was peformed on 30 males with non obstractive azoospermia. All patients were referred to the in vitro fertility department of Mirza Kouchak Khan Hospital, during 1998. The extracted sperm were injected in metaphase II of spouse oocytes. Depending on spermatogensis which has been observed on histology, all cases were diagnosed and categorized into three groups: 1-hypospermatogensis (17 males), 2-maturation arrest (8 males), and 3- pure sertoli-cell only (5 males). The mean age of males was 37, and for females 29 years. The mean FSH was 22.8 IU, and volume of the testis was 10 ± 5 cc. Sperm extraction from testicular biopsy were successfully retrieved in 21 males (70%). From 123 oocytes which were injected in metaphas II, 71 oocytes (57.7%) showed fertilization and were cleaved. The main sperm retrieval in hypospermatogenesis group was 15 males (71.4%), and in sperm maturation arrest group six males (27.7%). But in ," sertoli- cell group no sperm was extracted. From 21 cases with embryo transfer, clinical pregnancies were obtained in four cases (19%) and ongoing pregnancies in three Cases (14.3%). In conclusion we observed that physical examination, testicular volume determination, hormonal assay and testicular biopsy are essential but not sufficient in sperm extraction. Despite of the elementary negative biopsy, which showed impossible sperm extraction, but it was finnally extracted in 70% of patiants. So TESE/ICSI offers and must be considered as a new chance of pregnancy to these infertile couples.
