Abstract

Background: Hemophilia A is a bleeding disorder caused by defective production of factor VIII. In this hereditary disorder, the main concern is serious bleeding especially after trauma and surgeries. Factor VIII replacement therapy has decreased bleeding events during surgery substantially but there are some reports of thromboemblic events in this situation.Case Report: Reported here is a patient with moderate hemophilia A that had a catastrophic pulmonary embolism after splenectomy. The patient was a 25 year-old man with hemophilia A admitted after a car accident and trauma to left lower chest and abdomen. He received factor VIII concentrates for replacement therapy, but after 2 days due to abdominal tenderness he underwent laparatomy and then splenectomy. After surgery, he developed dyspnea and decline in blood pressure suddenly and died, thereafter autopsy of patient confirmed massive pulmonary thromboembolism.Conclusion: Although pulmonary thromboembolism in the early postoperative period in patients with hemophilia undergoing Surgeries and receiving factor VIII concentrate for replacement is rare but must be noted. In Surgeries such as Splenectomy, measures to prevent thrombosis such as administration of low molecular weight heparin and Regular factor VIII assay must be in attention.
