Seroprevalence and Risk Factors of Hepatitis E AmongWomen of Reproductive Age Residing in Birjand in 2016

Abstract

 Background: Hepatitis E is one of the most important health concerns in many developing countries. Several studies have shown that HEV is endemic in Iran. Hepatitis E is associated with high mortality in Women. Objectives: Due to scarce information about the prevalence of Hepatitis E in the population of Women of reproductive age residing in the city of Birjand, eastern Iran, the current study was conducted to evaluate the seroprevalence of Hepatitis E in Women of reproductive age. Methods: This descriptive-analytical study was performed in Birjand on 360 subjects older than 17 years who visited the Women’ s Clinic of Valiasr Hospital. The subjects were selected by the convenience sampling method. The data collected by questionnaires included personal information, history of diseases, marital and pregnancy status, and type of drinking water. Afterward, a 5-cc blood sample was taken from each subject and then, the titer of anti-HEV IgG was measured by the ELISA method. Results: The results were analyzed using SPSS-16 software by independent t-test. In addition, the odds ratios were estimated by the logistic regression model at a significance level of = 0. 05. This study was conducted on 360 subjects with the mean age of 31. 1  7. The minimum age of the subjects was 17 and their maximum age was 45. A total number of 105 subjects (29. 2%) were pregnant while 255 (70. 8%) were not pregnant. The Hepatitis E antibody titer was positive in 42 subjects (11. 7%) and negative in the rest. The prevalence of positive HEV was not significantly different between pregnant and non-pregnant, married and single, and employed and non-employed Women. However, a significant relationship was observed in its prevalence concerning the place of residence (P < 0. 001) and the type of drinking water (P < 0. 001). In addition, positive serology was lower in subjects with a college education than in subjects with lower education levels (P = 0. 004). The prevalence of positive antibody significantly increased with age (P = 0. 018). Conclusions: In the present study, the seroprevalence of HEV was 11. 7% in Women of reproductive age residing in Birjand city. The prevalence of Hepatitis E was higher in pregnant Women with low levels of education residing in rural areas. In addition, its prevalence was higher in subjects who drank untreated water. Therefore, the prevalence of Hepatitis E in rural areas can be reduced by providing education and promoting hygiene in these areas.
