LOCAL APPLICATION OF AMITRIPTYLINE IN COMPLETION OF LOCAL ANESTHESIA IN IRREVERSMLE PULPITIS PAIN

Abstract

 BACKGROUND AND OBJECTIVES: AMITRIPTYLINE, a tricyclic antidepressant agent and with binding sites of the Na+ channels can overlap with the local anesthetic receptors. To assess the additional analgesic efficacy of local administration of 2% AMITRIPTYLINE in IRREVERSIBLE PULPITIS pain.METHODS: A randomized, double-blind, placebo-controlled study of 56 consenting adult patient was done. Patients applied placebo and AMITRIPTYLINE 2% as maximum 0.2 ml injection in the pulp chamber after taking lidocaine LOCAL ANESTHESIA. Patients recorded overall pain using a 0-10 visual analogue scale during time of zero (before treatment), 1, 3, 5, 7 and 9 minutes of the study period.FINDINGS: Overall pain was significantly reduced by AMITRIPTYLINE, a reduction by 92.5% in 9 min compared to zero.This finding showed a significantly difference when compared to placebo (p<0.001). On the other hand, the average pain intensity at zero and 9 min was significantly reduced by AMITRIPTYLINE (8.6 to 0.65, p<0.01).CONCLUSION: Local application of AMITRIPTYLINE 2% produces analgesia and can complete lidocaine LOCAL ANESTHESIA.According to the results, it could be considered as an adjutant therapy in control of pain due to IRREVERSIBLE PULPITIS.
