PREVALENCE AND DETERMINING RISK FACTORS OF CLINICALLY UNDIAGNOSED OBSTRUCTIVE LUNG DISEASE IN INTERNAL MEDICINE WARDS (2005)

Abstract

 BACKGROUND AND OBJECTIVES: OBSTRUCTIVE LUNG DISEASE (OLD) is a great problem with major impacts on health and economics which may be frequently under recognized. The aim of this study was to determine the prevalence and risk factors of clinically-undignosed OLDs.METHODS: This descriptive and analytical study was performed on 200 patients who hospitalized in internal medicine wards at Babol Shahid Beheshti hospital during 2006. Patients completed a questionnaire and then SPIROMETRY was done. Patients with AIR FLOW LIMITATION were classified according to discharge diagnosis into two groups: clinically diagnosed OLD and clinically undiagnosed OLD.FINDINGS: One hundred and seventy patients with the mean age of 54±18 years consist of 98 males (58%) and 72 females (42%) had acceptable SPIROMETRY according to ATS (American Thoracic Society) criteria and entered the study.Air flow limitation was seen in the SPIROMETRY of 82 (48.2%) patients and 26 patients of this group (31.7%) were not recognized by the physicians at the time of hospital discharge. SPIROMETRY could increase diagnosis of OLD from 32.9% to 48.2% (p=0.000). OLD was more prevalent in men than women (p=0.005) and in age³45 years old more than<45 (p<0.025). Wheezing had a significant predictive value for diagnosis of OLD (p=0.000). Thrashing corn-dust, active smoking and opiates inhalation had a significant relationship with the spirometric diagnosed OLDs (p<0.042, p>0.000 and p>0.001, respectively).CONCLUSION: OLD is common but may undiagnosed in patients admitted to medical wards and SPIROMETRY should be considered clinical signs, history of cigarette or opiate smoking, environmental and occupational exposure.
