RESULTS OF SURGICAL TREATMENT FOR SPRENGLE'S DEFORMITY WITH VERTICAL CORRECTIVE SCAPULAR OSTEOTOMY

Abstract

 Background: Sprengle's deformity which is the result of an abnormally cephaled position of SCAPULA, is the most common congenital anomaly of shoulder complex. There are different surgical methods of treatment. This is a report of results with a corrective vertical SCAPULAr OSTEOTOMY during years 1997 to 2003 in Shafa Rehabilitation Hospital.Methods: In a retrospective review the results of the “Vertical SCAPULAr OSTEOTOMY” were evaluated in 23 consecutive patients with an average follow-up of 30 months (6 mo-6 years). There were 19 girls and 4 boys in this series, with 15 right, 6 left, and 2 bilateral cases. The age range was 2-13 years (mean age 7.5). The degree of correction was assessed by measurement of superior SCAPULAr angle (SSA) and inferior SCAPULAr angle (ISA). The improvement in shoulder motion as well as general appearance was clinically assessed.Results: all 23 osteotomies healed with mean improvement of 15.82 and 20.69 degrees for SSA and ISA respectively (p<0.001). The mean shoulder flexion and abduction improved 30.13 and 35.66 degrees respectively (p<0.001). Subjective satisfaction was claimed in all cases. The cases with omovertebral bone had better corrections in abduction motion. Resection of a wedging of bone from the lateral corner of SCAPULA was effective in improving SCAPULAr.Conclusion: Good functional improvement, less soft tissue manipulation with less bleeding, and also significant improvement in shoulder motion and SCAPULAr rotation make “vertical SCAPULAr OSTEOTOMY” and a better choice for Sprengle's deformity.
