A NEW METHOD FOR CLOSURE OF TOTAL LARYNGECTOMY'S WOUND

Abstract

 Background: FISTULA is one of the most complications after total LARYNGECTOMY. We innovate a new method that is simple and without complications specially pharangocutaneous FISTULA with this study we try to achieve an objective confirmation of the efficacy given by innovative method for closure of defect in total LARYNGECTOMY against the conventional method for closure.Materials and methods: Study was done as clinical trial, experimental type. 40 patients with SCC of larynx in T3 or T4 grade were chosen. All patients were done total LARYNGECTOMY by standard method, 20 patients (case group) were repaired by new method for closure and 20 patients were repaired by standard method (Control group). Lengths of operation and hospital stay, bleeding, hematoma, seruma and FISTULA formation were charted for every patient and statistically analyzed.Results: Min age of control group was 59.8±7.8 years and in case group 61.3±6.4 years. Length of operation in control group was 150±23.2 minutes and in case group 130±18.7 minutes (p<0.001). Length of hospital stay in control group was 10.5±1.6 days and in case group was 3.1±0.2 days (p<0.001). Bleeding, hematoma, seroma were not seen in any groups. Pharyngo cutaneous FISTULA was occurred in 3 patients in control group and none in case group.Conclusions: New method for closure would shorten the length of operation and hospital stay. Because the lack of FISTULA, we hadn't psychologic and traumatic side effects of tube feeding. This method had no complications. We suggest use of this innovative method as a simple and efficase for CLOSURE OF WOUNDs of total LARYNGECTOMY.
