COMPARISON OF PATIENTS ADMITTED DUE TO PERFORATED PEPTIC ULCER ACCORDING TO HISTORY OF CRACK ABUSE IN IMAM HOSSIEN HOSPITAL (2006-2007)

Abstract

 Background: Relation of CRACK abuse and peptic ulcer is presented from several years ago, now with increasing substances abuse, CRACK abuse is mentioned as one cause for increasing peptic ulcer arid its complications. In this survey we compared patients admitted due to PERFORATED PEPTIC ULCER according to history of CRACK abusing for better evaluation of them and reducing of miss diagnosis.Materials and Methods: In this study we evaluate 120 patients 119 male and 1 female admitted due to PERFORATED PEPTIC ULCER, we put them in two groups based on CRACK abusing history and compared them for pre, intra and post operative findings.Results: From 120 patients 42 (35%) had history of CRACK abuse and 78 (65%) had not this history, CXR was positive in 47.6% of CRACK abusers and in 76.9% of non abuse patients, WBC in CRACK abusers was about 11500 and in non abusers about 15000, perforations were mostly pyloric and perpyloric with sever contamination of peritoneal cavity in CRACK positives and gastric and deoudenal with less peritoneal contamination in CRACK negative patients.Conclusion: the results show that CRACK abuse patients are younger with less positive CXRs and leucocytosis despite of most peritoneal contamination, therefore evaluation of these patients should be with cautions.
