Abstract: 

Antifibrotic therapies should preferentially be targeted to the activatedhepatic mesenchymal cells. Those cells resemble wound healing myofibroblasts and synthesize an excess of extracellular matrix (ECM) proteins. They derive from quiescent hepatic stellate cells (HSC) and portal/perivascular (myo-) fibroblasts (MF). Whereas various agents have been shown to inhibit HSC/MF proliferation and collagen synthesis in vitro, only few of them are effective. Established in vivo models are rat secondary biliary fibrosis (chronic cholestatic liver disease) and reversion of fibrosis after withdrawal of a hepatotoxin like thioacetamide. The interferons (IFN-γ>α,β) have proven antiproliferative and fibrosuppressive activity on mesenchymal cells in culture. Retrospective data suggest that IFN-α therapy for hepatitis C can halt or even reverse fibrosis. This has to be confirmed by large randomized prospective studies, but an effect in biliary fibrosis is less probable. Strategies to inhibit the key profibrogenic cytokine TGF-β, e.g. by soluble decoy receptors, or molecules that are involved in TGF-β signal transduction are evolving but targeted approaches have to be used, in order to prevent unwanted side-effects. Novel agents are being developed in the form of orally available small molecule inhibitors (peptidomimetics). These include antagonists of certain integrins (ανβ3 or ανβ6) that are involved in HSC/MF migration or TGFβ-activation, respectively, or of the endothelin A receptor that causes contraction and proliferation of activated HSC/MF. Future studies will have to show if and how far drug combination is effective in man, combined with reasonable costs and no or irrelevant side-effects. Some agents are antioxidants like silymarin, a defined mixture of flavonoids, sho-saiko-to which contains related compounds like baicalein, glitazones, phosphodiesterase inhibitors like pentoxifylline, inhibitors of the renin-angiotensin system, halofuginone and some immunosuppressants like mycophenolate mofetil and rapamycin. Drug targeting to the fibrogenic liver cells is now possible by use of small molecular ligands that bind to receptors which are specifically upregulated on activated HSC/MF. These ligands can be exploited as carriers for otherwise toxic drugs, antisense DNA or siRNAs directed against profibrogenic mRNAs, such as those against procollagen I or TGFβ1. It is likely that liver fibrosis of different etiology will need different antifibrotic treatments. Quick progress in the clinical development of antifibrotic therapies can only be expected with the evolving validation of noninvasive, serological markers of fibrogenesis or fibrolysis.

Abstract: 

The relation between culture and language, features the need to examine the effectiveness of language textbooks. Considering the goal that foreign language teaching is mainly associated with the entry of cultural issues into the learner' knowledge, this need increases especially during learning a foreign language. Due to the today's growing importance of visual communication, this article offers a new perspective on images of English language textbooks in our system of education, based on the semiotic model of Kress and Van Leeuwen (2006), which emphasizes the importance of constituent elements of culture and society in analyzing linguistic signs; such as clothing, traditions or customs of cultures and subcultures. Paying attention to limitations of this case study, "ethnicity and location" are analyzed in the images of "Visions" and "Prospects". The present study explains how discourse is represented in these images, based on a scientific background and descriptive-analytical method with the aim of examining the social and cultural functions of language. The results show that the studied textbooks don't have a specific "ethnic-racial" approach and selection of "place" was without bias of the source language or the target language; In this way, the illustrator didn't pay special attention to his or her own language in choosing the images...

Abstract: 

The present study is an attempt to analyze the content of Prospect textbooks taught at the Iranian junior high schools according to the generative learning framework. Based on the four main processes of generative theory (motivation, learning, meaning making, generation), a checklist was developed. Four experts in TEFL and 21 English teachers were asked to analyze 9 selected lessons from the books. The content validity of the checklist was confirmed by the 4 experts and the intra-and inter-rater reliability indices showed a significant reliability level. Then, interval evaluation was used for the analysis of the obtained means. The results showed that the textbooks were evaluated as successful according to the generation criterion, moderate regarding meaning making criterion and weak in terms of learning and motivation criteria. Furthermore, the books were evaluated as weak regarding their generalizability power to non-learning contexts (which is an important aspect of communicative approaches) as well as motivation, developing thinking skills and raising awareness and attention criteria. Some suggestions are made for the improvement of generalizability of learning to out of-class contexts, thinking skills and motivation in texts, exercises and tasks.
