Abstract: 

Background & Aim: Medication errors are one of the most common medical errors. Since the performance of a theory in nursing is the extent of its application for description or prediction of evidence and also its potential in handling of the results as an occasional theory; this research was done to test the theoretical model of Reason human errors in nursing medication error.Materials and Methods: It was a descriptive, correlational study that was conducted at a large teaching hospital in Tehran, I.R. The nursing staffs (n=150) were chosen in different wards affiliated to Shahid Beheshti University of Medical Sciences through convenience sampling. A researcher made questionnaire was utilized to collect demographic data, Velasquez nursing care complexity scale, Salyer work dynamic scale, Gittel et al., nurse-physician communication scale, Minick et al., work commitment scale, Wakefield et al., reporting medication error scale and Rybowiak et al., learning climate questionnaire were used to test the hypotheses related to individual and organizational factors based on Reasons Human Error Model. Using SPSS-PC (v.16), and LISREL 8.8 the data were analyzed by descriptive and path analysis tests (P_ value<0.01).Results: The age of the majority of the respondents ranged between 31 to 40 years old (44.7%) and they had 10.60±7.24 years of work experience. Model fit indices (CFI=1, GFI=0.99, RSMEA=0.00, =4.83 RMR=0.65) showed that the paths considered for relationship between variables was appropriately predicted. Learning climate correlated with work commitment with standardized coefficient of (r=0.40) and learning climate correlated with physician - nurse communication with standardized coefficient of (r=0.20). There was no significant relationship between other variables.Conclusion: Based on the nursing medication error model, encouraging nurses to report error and using the model as a way to self-learning and other colleagues’ learning would be very helpful in the future. Also by creation of positive learning climate and modifying punishment for unintentional errors and proposing approaches to prevent recurrent errors in future occasions, mangers would be very influential.

Abstract: 

Background & Aim: Although burn wound and its pain management has gradually improved in recent years, but selection of an efficient dressing is yet a global problem and a major challenge for health care personnel. The aim of this study was comparing the effect of biological, synthetic and traditional dressing on burn wound pain in children.Material and Methods: This experimental study was conducted on 90 patients who were treated in burn ward of Sina health center in Tabriz city during 2008-2009. Patients were randomly assigned into three groups: synthetic dressing group with dressing changes every 3 days (30 patients), biological dressing group (30 patients) and traditional dressing group (30 patients), with daily dressing changes. Pain intensity (procedural pain and backgrounds pain), was evaluated using NIPS, CHEOPS and Wong Baker at days 3 and 6. Data was analyzed by descriptive (Mean, frequencies) and inferential statistics (Kruskal-wallis, Chi-square) using SPSSPC (v. 13).Results: Pain score during dressing change were compared among synthetic, biological and traditional dressing groups. Mean ranks of pain in synthetic dressing group was significantly lower than other groups, in third morning and night of hospitalization (procedural and background pain) (24.7 versus 56.1 and 53.5; 28.7 versus 54.5 and 51.3, respectively, P < 0.001) and in the morning of sixth day of hospitalization (procedural pain) (25.3 versus 45.2 and 51.6, respectively, P < 0.001).Conclusions: This study showed that synthetic dressing decreased pain and increased patient comfort because of reducing the frequency of dressing changes. The results confirm the efficacy of synthetic dressing for the treatment of partial and deep thickness burns. It provides insights and suggestions for future research on modern dressing for burn wounds treatment.

Abstract: 

Background & Aim: Nursing report has been considered as a framework and an important means for providing and evaluating nursing care. The aim of this study was to determine the effect of educating problem-oriented reporting on the characteristics of nursing report.Material & Methods: This quasi-experimental study was done on the nurses in Urmia Seiyedolshohada and Taleghani hospitals. Nurses with bachelor’s degree in nursing (n=60) were first recruited by convenience sampling method and then nurses in Seiyedolshohada hospital (n=30) and Taleghani hospital (n=30) were randomly assigned to experimental or control groups respectively. Problem-based reporting workshop was held for experimental group. Data was collected with a checklist and analyzed by descriptive statistics, t-test and chisquare using SPSS-PC (v. 16).Results: The findings showed a significant difference between the mean scores of nursing report of morning, afternoon and night shifts, between experimental and control groups after intervention (P<0.05). The results of Independent t-test showed a significant difference between the mean scores of total nursing report between experimental and control groups after intervention (P<0.0001).Conclusion: According to the findings, educating problem-based reporting has positive impact on the characteristics of nurse reporting. Accordingly using this method by nurses is suggested.

Abstract: 

Background & Aim: Appropriate family planning counseling for informed choice, and the right and responsible use of contraceptive methods is very important. Cultural -religious barriers can have an effect on family planning counseling. The aim of study was comparing managers’, staff’ and clients’ viewpoints about cultural - religious barriers in family planning counseling in health care centers of Isfahan in 2012.Material & Methods: This is a cross-sectional one-step three-group comparative descriptive study conducted on 295 subjects including 59 managers, 110 staff and 126 clients in health care centers of Isfahan in 2012. The Managers and staff were selected by census and the clients were recruited through simple random sampling. The data collection tool was a researcher made questionnaire including two sections of fertility and personal characteristics, and measurement of viewpoints. Data was analyzed by descriptive and inferential statistical tests (one-way ANOVA) using SPSS-PC (v.19).Results: The findings showed a significant difference between mean scores of viewpoints in three groups of managers, staff and clients concerning cultural -religious barriers of family planning counseling (p<0.001).Conclusion: The results pointed to a difference among the viewpoints of the three groups, as though the clients considered cultural -religious barriers less prominent and less effective compared to other groups. Therefore, it is necessary to launch group discussion sessions with attendance of the three groups with the aim of investigation of problems which would have affected the performance of the participants in the field of planning and running correct family planning counseling.

Abstract: 

Background & Aims: patient compliance with therapeutic regimen predicts successful treatment and reduces the severity of negative complications. The aim of this study was determination of relationship between adherence to therapeutic regimen and health related quality of life in hypertensive patients in 2012.Material & Methods: It was a descriptive- correlational study. The hypertensive patients (n=264) were recruited by convenience sampling. Data was collected by demographic sheet, and Adherence to Treatment tool and health-related quality of life questionnaire (SF-36). The setting was selected hospitals affiliated to Tehran and Iran University of Medical Sciences including Imam Khomeini Hospital, Hazrat Rasoul Akram Hospital and Shahid Rajai hospital. Data was analyzed by T-Test and ANOVA and Linear Regression using SPSS-PC (v.16).Results: Most of Participants were 40-60 year old males. In most of patients adherence to diet was relatively good (79.5%), compliance with drug regimen was good (48%) and activity was poor (71%) and adherence to therapeutic regimen was relatively good (82%). Mean score of quality of life among hypertensive patients was 55.33. There was a significant relationship between quality of life and adherence to treatment using Pearson correlation test (p=0.001, r=0.671).Conclusion: Regarding the relationship between adherence to therapeutic regimen and health- related quality of life in this sample, teaching hypertensive people to increase their adherence and their quality of life is suggested.
