Abstract:

Background: Carpal tunnel syndrome consists of symptoms secondary to median nerve dysfunction in carpal tunnel. Every factor causing decrease in capacity of the canal can trigger the complaints. Squaring of the wrist surface somehow the anteroposterior diameter becomes 750.0 smaller than the mediolateral one, is known as a risk factor.Purpose: This study intends to determine the relationship between wrist cross-section and carpal tunnel syndrome.Method and Materials: In this case-control study the wrist cross-section in 120 subjects (60 involved and 60 intact) was measured by caliper. Presence or absence of the syndrome had been justified through electromyography. Results and Conclusion: The results indicated that the ratio of the anteroposterior diameter to transverse one in the affected and the control groups were 0.67±0.42 and 0.63±0.51 respectively. Therefore there is a significant difference of 50/0 representing the relation of carpal tunnel syndrome with the wrist cross-section.

Abstract:

Aims: The widespread use of sulfur mustard (SM) as a chemical warfare agent has proved its long-lasting toxic effects. Therefore, all health professionals should have sufficient knowledge and be prepared for any such chemical attack. Transforming growth factor (TGF-b), has an important role in the lungs of patients with some of chronic airway diseases, especially with respect to airway remodeling in mustard lung. Airway remodeling is characterized by defective extracellular matrix (ECM) turnover. Fibroblasts have a central role in ECM turnover. The TGF-b provides intracellular signals to regulate ECM production. The purpose of this study: isolate and achieve primary airway fibroblast cells directly from lung biopsies of chemical injuries and control samples and detect abnormal gene expression of TGF-b1 and TGF-b2 in isolated fibroblast cells, to study destructive effects on the SM gas and understanding Molecular mechanisms of SM, with the aim of developing medical Assistance. Methods: we used cell culture as an in vitro model. Thus, we cultured primary airway fibroblasts from epibronchial biopsies, and investigated gene expression of TGF-b1 and TGF-b2 in primary fibroblasts of SM injured patients and controls. Expression of TGF-b variant and receptors was measured by Semiquantitative RT-PCR.Result: Our findings revealed that expression levels of TGF-b1 and TGF-b2 was upregulated in the airway Fibroblasts of SM exposed patients in comparison with control samples. Conclusion: Our novel data, suggested that over-expression of TGF-b molecule in primary airway fibroblasts of mustard gas injured patients may involve in progression of airway remodeling of these patients.

Abstract:

Drug interaction is one of the most important problems in the treatment of diseases. The concurrent prescription of different drug categories may affects the activity of each drug. The war-injured veterans suffering from different chronic intractable diseases are a group of patients with high risk of drug interaction. A cross-sectional study was conducted on war-injured veterans treated in a psychiatric hospital for 4 months. All drugs used by each patient were identified during an interview by a physician. Data were analyzed in SPSS software. The investigation was conducted on 1435 patients. The number of medicaments used by each person was between 2 to 21. Drug interactions were identified in 99.3% of patients. There were no drug interactions in only 9 patients. The number of drug interaction was ranged from 1 to 55 in each patient with the total number of drug interactions of 15, 426 in all patients. Based on FDA classification, severe drug interactions were identified in 1493 patients, whereas moderate and mild drug interaction were observed in 13015 and 930 patients, respectively. The most common type of drug interaction was observed in the patients who received different anti-depression drugs (47.5%). Among patients with severe drug interactions, the clinical investigations revealed that 85.37% of the patients were suffered from cardiovascular side effects, 58.14% from neuromuscular, 54.45% form central nervous system, 44.35% from autonomic, and 37.21% from gastrointestinal side effects. The results indicate the necessity and importance of devising some guidelines to prevent or at least decrease the drug interactions in war-injured veterans with psychiatric disorders.

Abstract:

Introduction Chronic complications due to chemical weapon exposure can negatively affect the quality of life. The aim of this study was to assess the health related quality of life (HRQOL) in people who had severe pulmonary complications due to the sulfur mustard gas exposure. Materials and Methods In this cross-sectional study all patients (292) with severe pulmonary complications due to chemical weapons exposure whose information thrived from the Veterans and Martyrs Affair Foundation (VMAF) database were studied. The quality of life was measured using the 36-item Short Form Health Survey (SF-36). Results The mean age of the patients was 48.8±7.8 years. The lowest scores on the SF-36 subscales were found to be the role limitations due to emotional problems (19.8±33.5), and general health (20.3±17.5), respectively. The highest scores reported in social functioning (42.9±26.2), and emotional well being (41.5±21.5), respectively. Quality of life in chemical warfare victims who had pulmonary problems was significantly lower than the general public (P<0.05). Regression analysis indicated the most potent affect on both physical and mental components for doing exercises. Conclusion The study findings suggest that chemical warfare victims with pulmonary complications suffer from poor health related quality of life and it is so necessary to improve quality of life in this population.
