Abstract: 

Background: Hypertension is one of the leading causes of mortality in the world. The aim of this study was to evaluate the relationship of plasma 25-hydroxyvitamin D levels with markers of metabolic syndrome and ambulatory blood pressure monitoring. Methods: This was a cross-sectional study on 171 adults (79 men) with primary hypertension and mean age of 50. 70 ± 10. 88 years, who did not take vitamin D supplement for the past 6 months. Subjects were divided into two groups as deficient (30 ng/ml ≥ ) and normal levels of vitamin D (30 ng/ml <) and compared for metabolic syndrome indices and 24-hour blood pressure monitoring parameters. Hypertension was defined based on Eighth Joint National Committee (JNC8) scale, and measured by an ambulatory blood pressure monitoring device. Ambulatory blood pressure monitoring parameters were defined as 24-hour blood pressure (mean during 24 hours), daytime (6-23 hours), and nighttime (23-6 hours) measurements. Finding: The mean vitamin D level was 30. 43 ± 4. 70 ng/ml, and 65. 5% of patients had vitamin D deficiency. Serum vitamin D levels had an inverse and significant relationship with office systolic blood pressure (P = 0. 001), and 24-hour, daytime, and nighttime systolic blood pressures (P < 0. 001). Vitamin D deficiency was significantly associated with higher levels of triglycerides (P < 0. 001) and cholesterol (P = 0. 016). Conclusion: Vitamin D deficiency was associated with higher systolic blood pressure, nighttime diastolic blood pressure, and impaired serum lipid profiles

Abstract: 

Background: Early surgery to remove dead tissue with early closure of burn wounds has now been identified as one of the most important factors contributing to the survival of patients with major burn injuries. This study, the efficiency and safety of temporary cover wound dressing with agicoat was compared with allograft (cadaver) in burned patients. Methods: In this randomized clinical trial study, hospitalized patients in the age group of 18-45 years with burning of 20-30 percent and grade 2 deep and 3, were divided into two groups of control and test. Treatment was carried out in the operating room after debridement, covered with wounds or cadaver wounds, and patients were also treated for signs of infection such as warmth, discoloration, redness of the fluid, and fever. Patients were evaluated in two groups, and the surgeon checked the need for re-debridement, and recorded it in the checklist. Findings: A total of 50 burn patients met the inclusion criteria. Debridement in cadaver group was significantly lower than that of Agicoat group (P = 0. 003). Mean pain score during the first to sixth days of surgery, and also on the ninth day of surgery was significantly lower in cadaver group (P < 0. 050). The mean daily morphine injected over 24 hours was significantly lower in the cadaver group as well (P < 0. 050). Conclusion: According to the findings of this study and other similar studies, it can be concluded that the use of cadaver reduces the need for debridement and morphine consumption in comparison with Agicoat

Abstract: 

Background: Having a child is the strongest desire in the world, and failure in it can be associated with stress, reduced interpersonal and social relationships, sexual dissatisfaction, and ultimately the breakdown of married life. The aim of this study was to evaluate sexual satisfaction and related factors in infertile couples with female infertility. Methods: This descriptive cross-sectional study was performed on 103 infertile couples (206 patients) referred to the infertility center of Tehran, Iran. The Data were collected by two questionnaires including demographic questionnaires and Larsons Sexual Satisfaction Questionnaire. Findings: The total score of Sexual Satisfaction Questionnaire was 75. 00 (low sexual satisfaction) for infertile women, and 79. 92 (moderate sexual satisfaction) for their husbands. Infertile women had significantly less sexual satisfaction than their husbands (P < 0. 001). Women's sexual satisfaction had a statistically significant relationship with their education level, age of referral to infertility center, and infertility history (P < 0. 001); and sexual satisfaction of infertile women's husbands had a statistically significant relationship with duration of infertility and age of marriage of women (P < 0. 001). Conclusion: The present study showed that infertile women had low sexual satisfaction, and their husbands had moderate sexual satisfaction. The sexual satisfaction of infertile women was significantly lower than their husbands, which requires special attention
