Abstract: 

Background and aims: Nurses are the most effective of groups who are present after incidence of disaster. So, they should provide requirement cares using skills and fast performance.Assessing competence before participation in this situation can promote their readiness. The aim of this study was to assess the disaster nurse competence in Isfahan educational hospital.Methods: In this cross-sectional study, 400 nurses were selected by census in 2013. Data were gathered by a questionnaire included 50 questions in NCDS (Nurse Competence Disaster Scale) in 4 domains. The data were analyzed using SPSS software and analytic and descriptive statistical tests.Results: The mean score of samples was (42.53±8.87) in management domain, (19.77±3.19) in ethical and legal domain, (32.73±6.11) in personal domain, (77.92±16.64) in technical and total score was (172.96±30.32).Conclusion: The findings of the current study showed nurses' competence for presenting care in disaster situations is unfavorable. So, it is necessary nurses to acquire specific and technical skills more than base skills that they occasionally received in educational courses to be able to act effective during care in disaster.

Abstract: 

Background and aims: Multiple sclerosis (MS) patients compared to healthy subjects have much higher levels of psychological disorders such as depression, anxiety and stress that greatly affect their quality of life and significant role in stimulating the onset of MS, worsening symptoms and increasing fatigue. Relaxation in complementary medicine is the most common treatments with low cost. This study aimed to determine the effects of relaxation on depression, anxiety, stress and in women suffering of MS.Methods: This study is a randomized clinical trial that was carried out on 50 patients with MS referred to MS Clinic of Kashani Hospital in Isfahan, Iran. After simple non-random sampling, participants were randomly assigned by minimization method to three groups: Reflexology, relaxation, and control groups. In the experimental group was performed relaxation intervention within 4 weeks, twice a week for 40 min and the control group were received care and routine medical treatment as directed by a doctor? Data were collected through DASS-21 questionnaire before, immediately and 2 months after interventions in two groups. Data analysis was performed by SPSS using descriptive and inferential statistics.Results: Mean scores of stress, anxiety, and depression before the intervention showed no significant difference in the two groups (P>0.5), but immediately after the intervention, independent t-test showed significant difference between mean scores of stress (P=0.001), anxiety (P=0.02) and depression (P=0.02) between groups while two months after the intervention there was only a significant difference between the mean score of stress in the two groups (P=0.003). Also, findings obtained from repeated measures analysis of variance showed that the severity stress, anxiety and depression during the different times in experimental group had significant difference (P<0.001); while, this difference was not significant in the control group (P>0.05).Conclusion: It seems that relaxation technique as intervention in reducing stress, anxiety and depression is effective in women with MS. However, the technique has long-term effects on stress. So, this method as effective technique can be recommended.

Abstract: 

Background and aims: According to the increasing prevalence of Chronic Obstructive Pulmonary Disease (COPD), and decreasing the quality of life these patients, the present study was aimed to investigate the effect of home visits on the quality of life of elderly suffering from COPD hospitalized in the Khalij Fars hospital of Bandar Abbas, Iran in 2015.Methods: The current quasi- experimental study was carried on in Khalij Fars hospital of Bandar Abbas, Iran on 62 elderly suffering from COPD with moderate intensity in 2015-2016 based on Spirometry results and the specialist`s recognition. The data collection tools were demographic questionnaire and quality of life questionnaire (SF-36). The data were analyzed by SPSS software, and using the independent T-test, and K2.Results: Based on results in the study, it was not observed a significant difference between two groups in terms of gender (P=0.793), marital status (P=0.998), habitation (P=0.704), education (P=0.145), and economic situation (P=0.605). There was a significant difference in the total score of the educated group compared to the none- educated group (P<0.0001), and the average score of the intervention groups before the test was 51.90 while this number increased to 66.73% after intervention. The rate of readmission was 66.7% in the control group and 57.4% in the case group. It shows that home visits have been effective in reducing re-hospitalization of elderly people (P=0.028).Conclusion: Regarding these findings, the education program along with home-visiting had improved the quality of life of elderly having Chronic Obstructive Pulmonary (COPD). Also, home visits aimed at maintaining health and independence of older people to prevent their disability after discharge from care facilities are considered a necessity.

Abstract: 

Background and aims: The prevalence of chronic diseases, such as diabetes mellitus, in elderly can be controlled with training self-control behaviors, especially in peer groups. Regarding to specific conditions and greater rate of complications in the elderly, the peer group self-care education is in specific importance among these patients. The present study was aimed to investigate the effect of peer group training on the self-care of elderly with diabetes in selected health centers of Isfahan.Methods: The present clinical trial compared 21 elderly patients with diabetes higher than 65 years old trained by a peer educator and 23 elderly patients received training by none peer group (trained by a researcher) for 4 months. The samples were randomly divided into two groups. The self-care behaviors were assessed by a questionnaire in 4 dimensions of diet, sport, glucose test, and diabetic drugs and the results were compared between groups.T-independent for statistical tests and to compare three times in a group was used ANOVA with repeated observations.Results: The total score of self-care in the test group increased significantly from 31±5.61 before intervention, to 42±7.01 after intervention and 35.55±5.11 one month after intervention, while the control group had no significant change. Different dimensions of the questionnaire were not statistically with a significant difference between groups, except in the field of sports was significant a month later intervention (P=0.05). There was a direct relationship between level of education and self-care score.Conclusion: According to the findings, training the elderly patients with diabetes by a peer group can promote their self-care. Therefore, training with a peer group is advised as an efficient method for improving self-care of elderly diabetic patients.

Abstract: 

Background and aims: Enuresis is one of the most common disorders and problematic. Diagnosis of enuresis occurs when the urine is given 2 times in a week, for at least 3 consecutive months. This study was performed to compare the effectiveness of medicinal and non-medicinal therapy to control primary nocturnal enuresis of school-age children.Methods: This study is the one blind clinical trial which has been done on 64 children suffering from enuresis in the 6-12-year-old children in school-age reffered to the Urology Specialized Clinic in Golestan hospital, Ahvaz Jundishapur University of Medical Sciences. Children were divided parochial randomized in two groups of medicinal therapy (n=32) and non-medicinal therapy (n=32). Data collection tools include demographic questionnaire, diary note and check list. The data analysis was used from descriptive and inferential statistical tests (T-test and K2) and SPSS software.Results: The results of the present study showed that the level of improvement during intervention (one and two months) had a statistically significant difference between two groups, pharmacological and non-medicinal treatment groups (P=0.001, P=0.005), respectively. Recovery level in the group of medicinal therapy was higher than the non-medicinal treatment one. After three months from the start intervention stage, statistically significant difference was not observed between two groups (P=0.112); But in the stage of one month after the end of intervention, statistically significant difference was observed between the two groups of medicinal and non-medicinal treatment and the rate of improvement was higher in the non-medicinal al therapy (P=0.009).Conclusion: Due to the effect of duration on the results of the present study, effectiveness of nonmedicinal therapy has been more than medicinal therapy. So, it is recommended, non-medicinal therapy in order to effect on the enuresis control of children to be performed in the longer term and continuous follow-up.

Abstract: 

Background and aims: Patients suffer from chronic diseases such as hemodialysis patients may experience mental disturbances, social isolation, loneliness, helplessness, and hopelessness. It can lead to decrease quality of life and increase their need to continuing care especially receiving social support. The present study was aimed to investigate the relationship between anxiety, depression, and perceived social support, in one hand, and quality of life, in the other hand, among hemodialysis patients.Methods: This study was a descriptive research with a correlational design that was carried out in Sina hospital in Tehran.84 hemodialysis patients were randomly selected via randomized sampling from. The respondents filled out Zimet et al.’s Perceived Social Support Scale, Hospital Anxiety Depression Scale, and Kidney Disease Quality of Life Short Form (KDQOL-SF). Data were analyzed using SPSS, descriptive statistics including frequency, percentage, mean and standard deviation, and inferential tests including Pearson Product Moment Correlation, One-Way ANOVA, and Independent Samples t-test.Results: The results indicated that the life quality of hemodialysis patients was low (46.90±12.56), and anxiety (10±3.22) and their depression (9.53±3.25) were high. Moreover, there was a statistically significant direct relationship between their perceived social support (r=-0.34, P<0.01) and their quality of life and a statistically significant indirect relationship between their anxiety (r=-0.51, P<0.01) and depression (r=-0.54, P<0.01) amount and their quality of life.Conclusion: The findings imply that the life quality of hemodialysis patients could be improved by increasing social support, enhancing the quality of care offered by nurses and the patients’families, and also early screening of anxiety and depression among hemodialysis patients in order to develop necessary interventions.

Abstract: 

Background and aims: Fatigue is the most common side effects caused by congestive heart failure that can be as a complaint debilitating and often goes unnoticed by the health care team.The aim of this study was to determine the correlation of fatigue severity with demographic and clinical characteristics of patients with congestive heart failure.Methods: This correlational and cross-sectional study was performed on 93 patients with heart failure who were selected using simple sampling method in Shariati hospital and Isfahan cardiovascular research center during 9 months in 2015. Data collection tools were Demographic Questionaire and Fatigue Severity Scale (FSS). Data were analyzed using SPSS software, descriptive and analytical statistics such as mean, standard deviation, independent t-test, Spearman and one way ANOVA.Results: 64 (68.8%) patients were male and 29 patients (31.2%) were women. The mean age of patients was 61.26±7.39 years. The mean of fatigue score was 5.52±1.4.82.8% of patients with congestive heart failure suffered from severe fatigue. There was a significant correlation between fatigue with different levels of education level and presence of other chronic diseases (P=0.032 and P=0.041).Conclusion: The findings suggest that the severity of fatigue among patients with chronic heart failure is high. So, perception and recognization of fatigue signs and symptoms and its effective factors for suitable administration of patients by health careers is in priority.

Abstract: 

Background and aims: Teamwork is a way of organizing activities in health and health care that provides a broad approach to health needs. Due to the key role of teamwork for care and nursing profession, the study of the motivational factors and job barriers has considerable importance. Thus, the aim of the present study was to determine the relationship between team work, job motivation and job stress among nurses.Methods: In this cross-sectional study, 200 nurses employed in hospitals affiliated to Kerman University of Medical Sciences, were selected through convenience sampling. Data were collected by a four-part questionnaire consisted of the demographic data, Hoegl and Gemuenden Team Work Questionnaire, Hackman and Oldham Job Motivation Questionnaire and Job Stress Questionnaire (HSE). Data were analyzed using SPSS software and statistical tests including Pearson's correlation coefficient, Independent T-test, ANOVA.Results: The findings suggest that there was a statistically significant negative correlation between team work with job stress (P<0.0001) and between job motivation and job strain (P<0.02. There was a statistically significant positive correlation between team work and job motivation (P<0.0009).Conclusion: According to the findings, to improve and promote the quality of clinical practice depends on team work, and it is facilitated with the planning for increasing job motivation and reducing job stress causes.
