Abstract: 

Background & Aims: Effective management of human resources in accordance with changes in the inside and outside environment of the health care system is essential and has a significant impact on costs, time spent, speed of service delivery, and overall quality of health services. The purpose of this study was to design a local model of human resource management for the private health sector in West Azerbaijan province in 2019. Materials & Methods: In this descriptive-analytical study, the Delphi method was used to evaluate the current status of human resource management in private health centers of West Azerbaijan province and to identify the effective components. A questionnaire was prepared by consulting experts. Cronbach's alpha coefficient and SPSS software were used to determine the reliability of the questionnaires. Then, through modeling structural equations by Smart PLS software, the impact of human resource management components was assessed. Results: After implementing the three Delphi stages, using the valuable opinions of experts, the effective components of human resource management in the health sector and their various factors were determined and the 4 components of maintaining effectiveness, attracting effectiveness, hiring effectiveness, and improving effectiveness were identified by impact rates. Conclusion: Factors affecting human resource management in the private health sector can be used as a pattern for development in the public sectors of the health system by managers to use scientific and principled processes to formulate strategies and policies of the organization and thus help to increase the productivity of its organization.

Abstract: 

Background & Aims: Bipolar disorder is a severe and chronic psychiatric disorder that leads to impaired occupational and social performance of patients. The patient experiences the negative consequences of the disease, and also their caregivers experience an excess caregiver burden during their care. Caregivers' sense of empowerment and self-efficacy can moderate the amount of caregiver burden imposed on them. The purpose of this study is to determine the relationship between self-efficacy and care pressure and its related factors in caregivers of patients with bipolar disorder. Materials & Methods: This cross-sectional-correlation study was carried out on 70 caregivers of patients with bipolar disorder referred to Razi Psychiatric Hospital in Urmia. The sampling method was Convenience Sampling. A standard caregiver burden questionnaire (Zarit) and Sherer's General Selfefficacy questionnaire were used to collect the required data. Data analysis was performed using SPSS22 software. Results: 67/1% of the caregivers were female. The mean score of caregiver burden and self-efficacy of the caregivers were 45/2 with standard deviation of 16/05 and 41/99 with standard deviation of 7/64, respectively. Caregiver Burden was moderate and severe for 58. 6% of the studied units. Also, 4. 3% of them had a good self-efficacy score. There was no significant relationship between caregiver burden and self-efficacy status of caregivers of patients with bipolar disorder. Conclusion: Generally, Caregivers of patients with bipolar disorder did not have adequate self-efficacy and caregiver burden. Therefore, planning by nurses to improve caregivers' self-efficacy can be effective in empowering them to manage conditions and problems related to bipolar patients.

Abstract: 

Background & Aims: Ensuring the quality of higher education, especially doctoral education, is one of the concerns and priorities of higher education planners. Adapting, comparing, and analyzing different dimensions of educational programs with other countries are very effective in addressing this concern. Therefore, this study tried to comparethe Iranian Nursing PhD program with the University of Toronto in Canada, UC Davis in California and Manchester in the United Kingdom. Materials & Methods: This descriptive-comparative study was conducted in 2020. The four-stage Beredy model of description, interpretation, neighborhood, and comparison was used to compare the Iranian nursing education system with the Nursing Faculty of Toronto in Canada, UC Davis in California and Manchester in the United Kingdom. Data were collected by the reputable websites of the universities. Then, the data were compared according to the aim of the research. Results: The program of all four universities has mission, vision, and values based on the principles of strategic planning. In Iran, entering the doctoral program requires an entrance exam. But this is not the case in other universities. Most face-to-face research methods and seminars are held at UC DAVIS. University of Iran and UC DAVIS in California provide financial support for students, but at the University of Manchester, official approval of student financial support and funding must be provided to the university. The University of Manchester has the most research time, but the other three universities offer face-to-face and research courses. The University of Manchester also offers part-time and distance learning courses. Conclusion: One of the strengths of the doctoral program in Iran is the compatibility of the values and beliefs of the program based on the system of Islamic values. However, it is not clinically oriented, and emphasizes only theory and research and requires the student to attend classes in person. Therefore, based on the experiences of developed countries and the needs of Iranian society, we should correct the weaknesses and strengthen the strengths.

Abstract: 

Background & Aims: Leukemia is the most common childhood tumor, accounting for approximately one-third of all cancers of this age. Children with leukemia often receive multimodal treatments, which can cause acute and long-term side effects. Painful procedures, long hospital stays, and unclear prognosis are common stressors that can affect these children's emotional and behavioral systems. This cross-sectional study was performed to determine the extent of behavioral-emotional problems in children with leukemia undergoing chemotherapy at Shahid Motahari Educational Center in Urmia, 2019. Materials & Methods: In this cross-sectional study, 163 children with leukemia (mean age 9. 27 ± 2. 82 years) who were referred to the oncology clinic of Shahid Motahari Hospital were selected using available sampling method. The demographic information questionnaire and the Child Behavior Checklist (CBCL) were used to collect data. After getting acquainted with the children and their parents, the purpose of the study was explained to them. After obtaining informed consent from the parents of all patients, the questionnaires were completed by the parents. The collected data were analyzed using descriptive and inferential statistical methods (independent t-test and one-way analysis of variance). Results: In this study, the mean and standard deviation of total, internalized, and externalized behavioral problems were 97. 92 ± 28. 24, 61. 9 ± 16. 51, and 25. 11 ± 11. 34, respectively Based on the normalized average, the highest score was related to the withdrawn/depression component (57. 81 out of 100). Statistical analysis of the data showed that the mean score of total behavioral problems (p = 0. 000) and all the subscales were significantly different among the participants with different levels of education (p<0. 05), but they had no significant relationship with gender, place of residence (city / village), number of children, parents' education, parents' occupation and family economic status (p>0. 05). In addition, the Pearson correlation coefficient showed that there was a statistically significant positive relationship between the mean score of total problems and quantitative variables of child's age, age of diagnosis, father's age and mother's age (p>0. 05). Conclusion: Based on results of this study, the scores of anxiety / depression, isolation / depression, attention problems and internalized behavioral problems were above average. The mean scores of total behavioral problems and their components in school children (the first year of high school) is much higher than the children of lower grades. Therefore, it is important to identify and prevent such cases in high school children with leukemia.

Abstract: 

Background & Aims: Investment in education of nurses can help improve performance and the quality of provided services. Training needs assessment is the right step to design appropriate, and cost-effective programs. This study aimed to assess the educational needs of nursing staff. Materials & Methods: This descriptive cross-sectional study was conducted on 150 nurses working in teaching hospitals of Kerman in 2019. Data were collected by a researcher-made questionnaire. This tool has five dimensions of nursing-related tasks (research/audit, communication/teamwork, clinical skills, administrative, managerial/supervisory). The face and content validity of the questionnaire were confirmed by four clinical nursing specialists. For reliability, Cronbach's reliability coefficient was 0. 89. Data were analyzed using descriptive statistics and Spearman and Kendall Tau correlation coefficient. Results: Most educational needs of nurses were dimensions of research and audit (9. 09), administrative, managerial/supervisory (4. 94), and communication/teamwork (3. 7). More than one-third of nurses (42%) needed training to design a research study and 30% needed training to assess patients' clinical needs. There was no significant relationship between any of the demographic-occupational characteristics of the participants and the dimensions studied (p<0. 05). Conclusion: The most important educational needs for nurses were in the field of research and audit, as well as managerial and supervisory tasks. The educational needs of nurses can guide design the educational content provided to nurses in the curricula offered at medical universities. Application of educational programs based on nurses' needs increases their skills in clinical, managerial, and communication fields, and thus improves healthcare and increases patient safety.

Abstract: 

Background & Aims: Hospitalization of children is a major cause of anxiety emergence in parents. The aim of this study was to determine the effect of family-centered care on parental anxiety of children with febrile seizure. Materials & Methods: In the present quasi-experimental study, using a convenience sampling method, 90 parents with children suffering from febrile seizure were divided into two groups: control (45 parents) and intervention groups (45 parents). Children in intervention group received family-centered care while the control group received normal care. Using the Spielberger State-Trait Anxiety Inventory questionnaire, mothers’ anxiety between 12 hours after hospitalization and one month after hospitalization was determined and compared together in two groups. Data were analyzed using descriptive statistics and a chi-square test, a Fisher's exact test via SPSS22. Results: The results showed that the anxiety score of parents at one month after hospitalization was 123. 18± 18. 66 in the control group and 106. 87± 28. 64 in the intervention group. This decrease in intervention group was significant compared to control group (p <0. 05). After the intervention, the total anxiety mean score (106. 87± 28. 64) decreased significantly in the experimental group compared to the baseline (133. 02± 15. 26) (p <0. 05). Conclusion: Family-centered care can help reduce maternal anxiety. Therefore, it is recommended to use this method in the care of hospitalized children.

Abstract: 

Background & Aim: Spirituality is a complex concept that affects the important life goals and is experienced by every human being. Comprehension of what people who are diagnosed with gender dysphoria understand about spirituality can help make sense of their individual and social needs and concerns. The purpose of this study is to fathom and explain the spiritual experiences of transgender people. Methods & Materials: This is a qualitative and phenomenological research study in the forensic field of the south of Iran. For this purpose, ten individuals with gender dysphoria (six Transfeminine, four Transmasculine) were selected by purposeful snowball sampling and in-depth and semi-structured interviews were utilized. Data were analyzed using theoretical coding. Results: In axial coding, while comparing categories and codes with each other, their relationship was examined and finally the general categories of Resort, Acceptance, Divine testing and Relaxation, and the main theme of the strong desire for spirituality were extracted from data analysis. Conclusion: The spiritual experiences of transgender people cover a wide range of principles. The participants in the study cited the feelings of insecurity and fear of loneliness as their main reasons for paying attention to spirituality. It can be concluded that in addition to the physical need, the spiritual dimension of transgender people's lives must be carefully considered to ensure their individual and social health.

Abstract: 

Background & Aims: The term sexual intercourse as a psychosocial sensation has been defined in relation to the individual's interpretation of how to express the existence of his/her feelings in marital relationships, and his/her feelings and thoughts about sexual attraction, sexual interest, and participation in sex and sexual performance self includes the quality of sexual life. The aim of this study was to determine the relationship between sexual assertiveness and sexual quality of life in married women. Materials & Methods: In this correlation study, among married women referring to health centers in Kerman, 500 subjects who were eligible based on the inclusion criteria, in 2017, were selected through available sampling. The research tools included the Hubert Sexual Assertiveness Index, Sexual Quality of Life, and Demographic Information Questionnaire. Data were analyzed by SPSS-19 software and using inferential statistics such as Pearson correlation, Spearman correlation, one-way ANOVA, linear regression, and multiple regression and a p-value less than 0. 05 was considered to be statistically significant. Results: The mean score of sexual assertiveness was 47. 09 and the mean score of sexual quality of life was 46. 46. There was a positive and significant correlation between sexual assertiveness and sexual quality of life (0. 487, r = 0. 001. ) Conclusion: Regarding the relationship between sexual assertiveness and sexual quality of life, it seems that the use of approaches to raise the level of female sexual assertiveness in premarital education classes and communication skills with a spouse should be taught to improve the quality of sexual life.
