Reason for General Practitioners’,Reluctance to Pursue Residency Programs in Iran: A Grounded Theory

Abstract

 Background: One of the new challenges confronting the Iranian health system is the growing reluctance to enter a specialty residency program. We sought to assess the causes of this problem Methods: In the year 2021, a qualitative study was done to assess theresidency entrance exam. Unstructured interviews were used to obtain data, anda grounded theory approach on general practitioners who hadprevious experience participating in residency exams were analyzed using open, axial, and selective coding. Finally, the underlying theoretic model was developed. Results: Results revealed that participants in residency entrance exams are not appealing to the general practitioner. The intervening elements summarized in the three themes namely structural conditions, university dysfunction, and immigration issues. Our study revealedwidespread disappointment and indifference with regard to entering a specialty residency program. This disappointment resulted in either abandoning residency program and continuing as general practitioner and later participating in an esthetic surgery orattempting immigration abroad. This pattern may eventually lead to and an insufficient number of specialists as well as loss of talentsand brain drain. Conclusion: Paying attention to the needs of medical students, making changes in Medical Education and updating curricula, creating hope and improving economic conditions, particularly financial support for physicians serving in deprived areas are important. An interdisciplinary and multidisciplinary approach should be advocated for reforms to Iran's institutionalized monopoly in the health sector.
