Abstract

Background: Definite treatment of INSULINOMA is surgical excision. Complications due to blindly exploration of tumor make it necessary to localize tumor preoperatively. As a matter of fact, there is controversy in preoperative evaluation and specific localization of INSULINOMA. Thus, we studied patients with INSULINOMA referring to Taleghani hospital in 1998-99. Materials and methods: This retrospective study was performed by data obtained from all complete files in archives of hospital and included computing demographic information and clinical and paraclinical examination and operative information. PPV of different diagnostic approaches was determined. Results: There were 10 patients with INSULINOMA underwent surgery; 5 males and 5 females with mean age of 39.7:1:8.2 years. In this study, the determined PPV of each diagnostic approach was as below: abdominal sonography %10, abdominal CT scan %60, endoscopic US%0, intraoperative US %25, palpation of pancreas during operation %90. Conclusion: The only acceptable and reliable technique was palpation of tumor intraoperatively. Despite worldwide reports of EUS success, it was not a reliable approach in localizing the INSULINOMA, of course, this could be due to technical inadequacy.
