EVALUATING THE EFFECT OF REMIFENTANIL INFUSION VS. SINGLE DOSE INTRAVENOUS HYDRALAZINE PLUS PROPRANOLOL TO PROVIDE CONTROLLED HYPOTENSION IN PATIENTS UNDERGOING RHINOPLASTY IN KHALILI HOSPITAL OF SHIRAZ

Abstract

 Aim and Background: Bleeding during RHINOPLASTY has direct effect on the results of this operation, so every effort to reduce this would be appreciated. This study was performed to compare the result of using two different drugs in order to reduce bleeding during hinoplasty.Methods and Materials: This was a double-blinded study in which 80 patients with physical status class (ASA) I undergoing RHINOPLASTY were included. They were randomly assigned into two groups of 40: Group A received single dose of intravenous HYDRALAZINE plus PROPRANOLOL and group B was under REMIFENTANIL infusion during surgery. Patients’ blood pressure and heart rate and also the surgeons’ satisfaction were all evaluated. Data was finally analyzed using SPSS software.Findings: The demographic data and the surgical-anesthetic situations were similar between the groups.Surgeon’s satisfaction was significantly higher in the REMIFENTANIL group (p value<0.001). The mean time of surgery was longer in Remifentnil group.Also blood pressure was significantly lower in this group (p=0.036). However heart rate was not significantly different among the two groups (p=0.156).Conclusions: Our study shows that REMIFENTANIL infusion has better effect on surgeon’s satisfaction and blood pressure control during RHINOPLASTY than single dose HYDRALAZINE and PROPRANOLOL.
