EVALUATING THE EFFECT OF ADDING KETAMINE AND NEOSTIGMINE TO 0.25 % BUPIVACAINE FOR EPIDURAL ANALGESIA IN PATIENTS CANDIDATE FOR ELECTIVE HIP SURGERY

Abstract

 Aims and background: Pain is a complex medical problem and its inadequate control postoperatively has adverse effects on patients of physiological, metabolic and mental status. Adding new supplements will probably lead to an increased duration of analgesia. The purpose of this study was to evaluate the addition of NEOSTIGMINE and KETAMINE to bupivacaine 0.25% for EPIDURAL analgesia in order to increase duration of POSTOPERATIVE ANALGESIA.Methods and Materials: In this double blind clinical trial, 90 patients over 50 years candidate for elective hip surgery with ASA class I, and II were randomly divided to three groups: NEOSTIGMINE, KETAMINE and control groups. All patients received EPIDURAL Anesthesia with bupivacaine 0.25% by 2cc/segment.Furthermore60 microgram NEOSTIGMINE was added in the first group and 40 mg KETAMINE in the second group. Level of postoperative pain based on Visual Analogue Score (VAS), duration of analgesia, and amount of analgesic further requested by patients were compared among the three groups.Findings: The mean pain score at 6 and 12 hours after surgery was significantly lower in the KETAMINE group than the other groups and in NEOSTIGMINE group it was less than placebo (P£0.01). The mean duration of POSTOPERATIVE ANALGESIA in the KETAMINE group was significantly higher than the other groups and in NEOSTIGMINE group it was more than placebo (P£0.01). The mean dose of analgesic (pethedine) request was the least in the KETAMINE group (P£0.001).Conclusions: NEOSTIGMINE and KETAMINE added to bupivacaine (0.25%) for EPIDURAL anesthesia increase the duration of analgesia postoperatively and they reduce analgesic consumption which would be more with KETAMINE than NEOSTIGMINE.
