MESS AND PREDICTION OF COMPLICATIONS AND OUTCOME OF VASCULAR REPAIR OPERATIONS

Abstract

 Appropriate management of extremity injuries results in limb salvage but in severe injuries such attempts may cause several complications. These complications are due to restoration of perfusion to ischemic tissues. For better prediction of complications preoperatively we designe a descriptive - analytic study by preoperative calculating of Mangled Extremity Severity Score (MESS) and evaluate all patients operated due to extremity vascular injuries during 1993-2001. MESS was calculated by measuring scores of skeletal I soft tissue injuries, limb ischemia, shock and age. Then patients divided into two groups: one with scores less than 7 and another with scores equal or greater than 7. Relative frequencies were calculated for certain complications in each group and then the results were compared and analyzed statistically by Fisher's Exacts Test. Sixty eight patients (54 MESS<7 and 14>7) were included in this study. Relative frequencies of intraoperative complications including: hypotention, need to vasopressors, and acid base disturbance and postoperative complications including: mechanical ventilation, delay in extubation, limb reoperation, mortality and cardiac arrest and mean of bicarbonate and blood that infused intraoperatively and mean of BUN and creatinine (24 hours after operation) and duration of hospitalization were greater in MESS>7 group. There were no statistical differences in mean of volume of fluids infused intraoperatively. Thus relative frequency of complications in patients with MESS>7 was significantly more than those with MESS< 7 and limb salvage efforts, when not indicated, caused more severe complications and poorer prognosis.
