EVALUATION OF EFFECTS OF INTRAVENOUS DEXAMETHAZONE ON PREVENTION OF LARYNGOSPASM AFTER EXTUBATION IN CHILDREN

Abstract

 Objective: LARYNGOSPASM is a common condition in CHILDREN after extubation, as it is a critical medical condition, it may threaten patient’s life rapidly And become problematical for Anaesthesiologists. On the other side, intravenous DEXAMETHASONE has many advantageous and carries few complications during ANESTHESIA. In this study we try to research the effects of DEXAMETHASONE on prevention of LARYNGOSPASM after extubation in CHILDREN in comparison with placebo.Matrials and methods: In this double-blind clinical trial, 90 of CHILDREN who were candidate for elective surgery were chosen randomly and divided to 2 equal groups. DEXAMETHASONE (0.5mg /kg) and placebo were injected intravenously (using coded syringes) to the patient in equal volumes. Results were compared by the use of spss ver 15 software and mann- whitney U test.Results: no LARYNGOSPASM seen in the group with DEXAMETHASONE injection while 6 people of placebo group experienced LARYNGOSPASM after extubation. (14%, P value=0.026).Conclusion: regarding the effect of single dose of DEXAMETHASONE on prevention of subsequent LARYNGOSPASM in CHILDREN during general ANESTHESIA, it is suggested that all CHILDREN who are going under general ANESTHESIA where it needs endotracheal intubation, should receive single dose of DEXAMETHASONE 0.5mg/kg Iv up to 10mg after induction of ANESTHESIA.
