THE EFFICACY OF THE ADDITION OF FENTANYL TO BUPIVACAINE FOR LUMBAR EPIDURAL ANALGESIA DURING LABOR

Abstract

 Background and Objectives: Regional analgesic techniques for labor that permit analgesia are of great assistance among parturients. This study evaluated the efficacy of the addition of epidural FENTANYL to local anesthetic in an attempt to prolong analgesia during labor. Materiols and Methods: In a double-blind, clinical trail of 40 ASA (American Association of Anesthesia) physical status I, nulliparous and multiparous parturients, at term were taken into study. After infusing 500-1000ml of IV solution, continuous lumbar epidural technique was performed in an active phase, and one of the epidural study solutions was administered: 10ml of 0.25% BUPIVACAINE (n=20) (Group I) or 10ml of 0.125% BUPIVACAINE plus 1µg/ml (10µg) FENTANYL (n=20) (Group II). In case she had pain, above doses via the epidural catheter were repeated. Results: The onset of analgesic effect, intensity and duration of analgesia were significantly different among two groups: the mean onset of analgesic effect in group II was faster than group I (P=0.23). Verbal scales were significantly higher in group I parturients (P=0.02). Duration of LABOR ANALGESIA provided to group II was 148±:27min, and to group I 90±34min (P<0.001). Hypotension was observed in both groups, but occurred transiently in group II (P=0.014). Significant differences were not observed in the sensory block level, motor blockade, O2 saturation, and fetal heart rate as well as neonatal Apgar score parameters in any patient of both groups. Conclusion: The addition of FENTANYL (10µg) to low dose of BUPIVACAINE can significantly improve analgesia parameters without adverse maternal or fetal/neonatal effects.
