EFFECTS OF ADDING NEOSTIGMINE TO LIDOCAINE IN INTRAVENOUS REGIONAL ANESTHESIA ON SENSORY AND MOTOR BLOCK DURATIONS AND ANALGESIC REQUIREMENT AFTER HAND SURGERIES

Abstract

 Background: The acetylcholinesterase inhibitor, NEOSTIGMINE has been shown peripherally mediated analgesic action. The aim of this study was to evaluate the effects of NEOSTIGMINE added to LIDOCAINE for IV regional anesthesia (IVRA) on sensory and MOTOR BLOCK ONSET and recovery and time to first analgesic requirement. Materials and Methods: In a placebo controlled, double blind randomized clinical trail, 72 ASA class I & II patients undergoing hand surgery in Dr. Shariaty Hospital complex during 1382-83, were randomly assigned into two groups to receive IVRA. The control group received 1 ml of saline plus 4 mg/g of LIDOCAINE; the study group received 0.5 mg of NEOSTIGMINE (1 ml) plus 4 mg/g of LIDOCAINE. In both groups injection solution were diluted with saline to a total of 40 ml. Sensory and MOTOR BLOCK ONSET and recovery, anesthesia quality determined by an anesthesiologist and operative field quality determined by a surgeon were noted. Heart rate, systolic, diastolic and mean arterial blood pressure and oxygen saturation values before tourniquet and at 5, 10, 15, 20, and 40 min after tourniquet inflation and time to first analgesic requirement, after tourniquet release, were noted. Data were analyzed using SPSS for windows and Mann-Whitney and X2 tests. P<0.05 were considered significant.Results: In the NEOSTIGMINE group, sensory and MOTOR BLOCK ONSET times were significantly shorter and sensory and MOTOR BLOCK RECOVERY times were significantly prolonged (P=0.001).There were no significant differences between quality of anesthesia and operative field quality, heart rate, systolic, diastolic and mean arterial blood pressure, and oxygen saturation values at all measured times (P>0.05); time to first analgesic requirement was significantly prolonged in the NEOSTIGMINE group (P=0.001).Conclusion: NEOSTIGMINE has been added to local anesthetics for IVRA shorten the sensory and MOTOR BLOCK ONSET times and prolonged recovery times and time to first analgesic requirement.
