Abstract

In order to create a transform in Medical Services INSURANCE system, it is necessary to carry out a financial analysis on the medical costs. For this reason, an applicable (or feasible) prospective study was performed through an Analytic- Descriptive method, on 1081 . patients were admitted in one of the non-contracted hospitals in the year 1377 and were referred to the INSURANCE Administration to reclaim their expenses. The Medical Services items and also the expenses were statistically analyzod by SPSS software and also tested by Chi-Square and T- test based on the hospital illustrated that the average admission cost of the patients with the average 4.67 days of hospitalization was 2070046 Rials. On the other hand the average cost of treatment was 2070046 Rials, based on private tariff and 697539 Rials, according to the GOVERNMENTAL TARIFF, which the Medical Services INSURANCE Administration has paid the %90 of the costs, based on GOVERNMENTAL TARIFF that is equal to 627785 Rials, so that the %30.3 of this amount has been paid by the patient. And also there was a meaningful difference between the payments for the male and female insured patients, based on the approved GOVERNMENTAL TARIFFs (p<00.00003). And the FINANCIAL CONTRIBUTION of the insured patients in the costs payment was 69.7 % of the self employees coffer (Sandogh khishfarmayan) which the average treatment cost was equal to 3151414 Rials and just %2.4 were under coverage of the INSURANCE coffer of the Other Levels (sayer aghshar) And %86.5 of the patients believes that the Medical treatment costs are high so that they hardly could afford it. The results show that the patients pay the major of their medical costs by themselves (%70), so in this way the Medical INSURANCE is far away of the real meaning of INSURANCE and its standards.
