Effect of Team Self-Management Training on Blood Sugar Control in Children and Adolescents with Type I Diabetes

Abstract

 Introduction: Since diabetes type I is a chronic disease without definitive treatment, one of the main goals of the health system is increasing patients’ self-empowerment to control blood sugar and managing the disease. With regard to the limited number of studies carried out on this age group, the current study aimed to investigate the effect of team Self-Management training on blood sugar control in Children and Adolescents with Type I Diabetes. Materials and Methods: This quasi-experimental study was conducted on 32 patients with Type I Diabetes within the age range of 6-18 years. Initially, hemoglobin A1c (HbA1c) was measured and Self-Management training was delivered by the treatment team. The patients were asked to record the causes of increasing and decreasing blood sugar and its levels. This program lasted for 3 months in six sessions. Finally, HbA1c level, fluctuations in blood sugar, frequency of hypoglycemia and hospitalization, and daily insulin dose were compared before and after the training. The data were analyzed using SPSS software (version 18). Results: The mean age of the participants was 12. 59± 0. 5 years. The mean values of HbA1c were 10± 96. 40 and 9± 85. 35 mg/dl before and after the training, respectively. This mean difference was significant (P<0. 05). After the training, short-acting insulin dose significantly decreased (P<0. 05), and long-acting insulin dose increased (P<0. 05). The frequency of hospitalization and hypoglycemia showed no significant difference before and after the study (P>0. 05). Conclusion: Since Self-Management training had an effective role in the reduction of HbA1c and used insulin dose, it is suggested to provide the training in the care programs for type I diabetic patients in order to better control blood sugar and decrease insulin requirement.
