Abstract

 Purpose: To assess the safety of corneal epithelial removal by transepithelial phototherapeutic keratectomy (PTK) compared to MANUAL DEBRIDEMENT prior to PRK. Method: This single masked, clinical trial was conducted on 48 eyes of 24 patients. One eye of each patient underwent epithelial removal by transepithelial PTK prior to PRK with a Nidek Ec5000 excimer laser and the other eye underwent manual debridment. The results were compared according to spherical equivalent (SE), uncorrected visual acuity (UCV A), best-corrected visual acuity (BCV A), haziness, and rate of re-epithelialization. Results: The mean SE prior to PRK in the manual group was -2.87±0.77 diopters (D) and in the laser removal group was -3.03D±0.77D. Six months after operation, mean SE in the manual group was 0.10± 0.53D and in the laser removal group was -0.13D±1.l5D (P=0.38). Mean regression from 1st to 6th month was 0.53D and 1.47D, respectively for the manual group and the laser removal group (P=0.00). In the manual group, 2 eyes (8.3%) and in the laser removal group, 10 eyes (41.6 %) lost one line or more of BCVA at the 6-month visit. In the manual group, 16 eyes (66.7%) and in the laser removal group; 5 eyes (20.8%) gained UCVA >= 10/10 at 6 months after PRK. Mean haze at 6 months after PRK in the manual group was 0.23±0.25 and in the laser removal group was 0.77+ 0.36 (P=0.00). Mean healing time of epithelial defect in the manual group was 3.17±0.48 days and in the laser removal group was 4±0.83 days (P=0.00). Conclusion: According to our study, transepithelial PTK can not be a good alternative for MANUAL DEBRIDEMENT, with regard to less safety, efficacy, and predictability.
