COMPARISON OF VISUAL OUTCOMES AND COMPLICATIONS OF PENETRATING KERATOPLASTY AND DEEP LAMELLAR KERATOPLASTY IN ADVANCED AND SEVERE KERATOCONUS

Abstract

 Purpose: To compare the visual outcomes and complications of PENETRATING KERATOPLASTY (PK) and DEEP LAMELLAR KERATOPLASTY (DLK) in cases of advanced and SEVERE KERATOCONUS (KCN).Materials and methods: This retrospective comparative study was performed on 48 eyes of 45 patients with advanced and severe KCN that underwent PK or DLK. Main outcome measures were visual acuity, astigmatism, graft survival, and complications. Result: We performed PK in 35 eyes of 32 patients and DLK in 13 eyes of 13 patients. Mean follow-up in PK and DLK groups was 31 ±17.6 months and 25± 16 months, respectively (P> 0.05). Mean pre-operative and last visit (at least 3 months after suture removal) BCVA was 1.05 and 0.118 LogMAR in the PK group and 1.160 and 0.169 LogMAR in DLK group. There was no statistically significant difference in mean UCVA, BCVA, donor diameters, recipient bed diameters, postoperative corneal power, and astigmatism, between PK and DLK groups. There was one endothelial rejection in the PK group. All corneas were clear at the last visit.Conclusion: Visual outcomes are similar in PK and DLK. DLK is a safe and effective procedure, because it is an extraocular procedure and has less rejection complications and follow up is easier and shorter. We propose DLK for advanced and severe KCN without central corneal scarring.
