Abstract

Supracondylar fracture of humerus, (Extension type III) is a common fracture in children which commonly needs surgical intervention. There are several approaches including Posterior, lateral, etc for open reduction and internal fixation (ORIF) of this fracture.For evaluation of the results of posterior and lateral approaches, a study was performed in two sex and age matched groups of patients. Follow up of patients was at least 3 months. Elbow ROM (Range of Motion) and loss of extension and flexion were measured clinically. There were 19 patients in the 1st group (Posterior approach) and 15 patients in the 2nd group (Lateral approach) for final follow up. The mean ROM in the 1st group (108±2.9) was significantly less than the 2 nd group (132±2) (P<0.001). Despite longer follow up time in 1st group (9.4 months in comparison to 5.5 months) there was more disability from loss of functional ROM in this group. Fixation of fragments with two lateral pins in the 2nd group didn't result in any change in carrying angle.This study suggests that surgery with lateral approach has better results and patients return have activity sooner.
