Abstract

 Introduction. As we know desl110pressinhas antidiuretic effect so it may be effective in renal colic by decrease the pressure in collecting duct. This study was done to assess the efficacy of desmopressin nasal spray compared with diclofenac given intramusculary in patients with acute renal colic.Methods. From march 2001 to october 2002, 90 patients that were admitted in two hospitals affiliated to Isfahan University of Medical Sciences, randomized into three different groups: group A received desmopressin (40µg - nasal spray), group B diclofenac (voltaren) 75 mg  intramuscularly and group C both desmopressin and diclofenac. Pain was assessed using a visual analogue scale at baseline, 10, 20 and 30 minutes after administring the treatments.Results. On admission, the severity of pain, age and sex distribution were simillar in three groups. % 56.6, %86.6 and 90% of patients in group A, B and C showed either complete releif or relative pain improvement respectively. At 10 min the pain decreased in all three groups. There was no significant difference between group A and B and B and C, but significant difference was noted between group A and C (P<0.05). At 20 and 30 minutes, there was significant difference between group A and B and A and C (P<0.05), but no difference between group Band C Discussion. Desmopressin has several advantages, e.g. ease of administration and apparent lack of side - effects which makes it suitable for ambulatory use. Desmopressin acts rapidly and seems to be effective in both single and combined therapy with diclofenac and may increase the analgesic effect of diclofenac.
