KIDNEY TRANSPLANTATION IN PATIENTS WITH ABNORMAL BLADDER

Abstract

 In this article we present our 11 years experience on patients with RENAL FAILURE that had abnormal bladder and after proper interventions RENAL TRANSPLANTation was performed. Between 1989 and first mid of2001 from 550 RENAL TRANSPLANTation were performed in our center by an unit team. 14 recipients were in average of 15 years old (9-42 years old) with lower urinary tract disorders (consist of2 patients with PRUNE BELLY, 2 patients with posterior urethal valve (PUV), 10 patients with NEUROGENIC BLADDER).Diagnostic procedures of these patients were sonography, voiding cystourethrography (VCUG), urodynamic and cystoscopy studies. In 2 patients with PUV, therapeutic interventions were valve ablation and convert of previous diversion, RENAL TRANSPLANT was done 6 months after in one patient and 9 months after in other patient. 2 patients with prone belly were operated reduction CYSTOPLASTY coincident with RENAL TRANSPLANT. For 10 patients with NEUROGENIC BLADDER we did some conservative methods like filling the bladder with normal Saline (in anuric patients) or using anticholinergic and SIC (Strile Intermittent Catheterization). 4 patients were responded but for 3 patients had done enterocystoplasty and for3 patients we performed ureterocystoplasty. For analysis of graft and patient survival we used Kaplan-Mayer curve.We don't have any mortality during and 3 months after the operation. The patients, who treated by anticholinergic drug +SIC, one or several time had fibrile urinary infection but they didn't have any damage of RENAL TRANSPLANT or hydronephrosis. All the patients that we had done entrocystoplasty used SIC for urine drinage. On patient with ileocystoplasty, recurrent treatable infections were created but on patient with ileosecocystoplasty urinary infections resistant to antibiotic treatment, who unfortunately was died because of septicemia after 3 years of RENAL TRANSPLANT. Urinary incontinence was continued on patient with ileosecocystoplasty patient. 3 patients with ureterocystoplasty void completely without need to SIC. One of the 2 patients with PRUNE BELLY involved to gradual increasing creatinine but low degree and in other patient we doesn't have any specific problem. During the study overall survival of patient was 93% and graft 85.7%.We recommend RENAL TRANSPLANT on patients with lower urinary tract disorders at least after 3-6 months of the reconstructive surgery.In our study the best results and least complications were on patients with NEUROGENIC BLADDER who were treated by ureterocystoplasty. We recommend this method to all patients who need to CYSTOPLASTY and have a nonfunction kidney with megaureter.
