COMPARING THE EFFECTS OF LIDOCAINE + DEXAMETHASONE + CRYSTALLOID SOLUTION REGIMEN AND SOLITARY CRYSTALLOID SOLUTION REGIMEN IN THE PREVENTION OF POSTOPERATIVE NAUSEA AND VOMITING IN STRABISMUS SURGERY UNDER GENERAL ANESTHESIA

Abstract

 Background & purpose: Following STRABISMUS SURGERY, there is a high incidence of nausea and vomiting which complicates the postoperative period. This study was designed to compare the effects of LIDOCAINE + DEXAMETHASONE + CRYSTALLOID SOLUTION regimen and solitary CRYSTALLOID SOLUTION regimen in the prevention of postoperative nausea and vomiting in STRABISMUS SURGERY under GENERAL ANESTHESIA.Design: Comparative, clinical trial study. Interventions: In a clinical trial study, 200 patients (Inpatient) with physical status 1 (without considering age and gender) that were admited to Farabi Eye Hospital for STRABISMUS SURGERY were randomly assigned to two equal groups. The experimental group was treated with preservative free LIDOCAINE (1.5 mg/kg) + DEXAMETHASONE(100 mcg/kg)+ CRYSTALLOID SOLUTION. In adults, Ringer solution was utilized at the rate of 1 cc/kg/h to replace insensible fluid losses and 2 cc/kg/h during the intraoperative period. In children, however we used the same solution at the rate of 0.5cc/kglh to replace insensible fluid losses and 4/2/1 formula (According to it hourly fluid requirements of 4cc/kg were employed for children up to 10kg , an additional 2cc1kg for each kilogram above 10 kg up to 20 kg , and a furture additional of 1 cc/kg for each kilogram above 20 kg) during the intraoperative period. The control group was treated with CRYSTALLOID SOLUTION (as experimental group) regimen. All patients received GENERAL ANESTHESIA employing fentanyl 2 mcg/kg intravenously as premedication, sodium thiopentone 5mg/kg and succinylcholine 1.5mg/kg intravenously for induction and halothane 1.5% (without N20)for maintenance of anesthesia with spontaneous ventilation during the operation. After the end of surgery, in the recovery room, in both groups, the feeling or episodes of nausea and vomiting were recorded.Results: In experimental group, we had one case of nausea and vomiting in the postoperative period (1% and p<0.05). In the control group, we had 4 cases of nausea and vomiting (4% and p<0.05) making the p.value significant in both the groups. The need for an antiemetic to treat this problem however did not arise. We kept them under close observation and discharged them when the symptoms abated. In both groups, we found no pain.Conclusion: We concluded that in the experimental group we had a very low incidence of postoperative nausea and vomiting and felt that CRYSTALLOID SOLUTION together with DEXAMETHASONE and LIDOCAINE could prove to be a valuable armamentarium to control postoperative nausea and vomiting following STRABISMUS SURGERY under GENERAL ANESTHESIA.
