INDICATIONS OF CORONARY ANGIOGRAPHY BEFORE VALVULAR HEART SURGERY

Abstract

 Incidence of coronary involvement regarding the etiology of valvular disease and also indications of coronary angiography before valve surgery is different in this group of patients. For aviodance of unnecessary coronary angiography, we decided to perform this study in our center to determine the incidence of CORONARY DISEASE and guidance of angiography before valvular heart surgery . In this retrospective study on 100 patients in Golestan hospitai of Ahwaz university of medical sciences, between Mars 1997 to Mars 2000. Results of coronary angiography before valve surgery showed that only 8 patients had significant coronay lesion (incidence of CORONARY DISEASE was 8%) with mean age 52.75 year (between 40 to 65 years old). Five of these patients were male (62.5%) and three of them were female (37.5%). Three patients with coronary disease had history of chest pain and two patients had history of smoking. One , three and fourt of the coronary patients had mitral ,aortic and multivalvular involvement respectively.Two of the coronary paitents without history of chest pain or coronary risk factors, had aortic valve disease. According to the findings of this study , the most important predictor factors of coronary disease before valvular surgery were history of chest pain and presence of risk factors for coronary artery disease, thus coronary angiography is unnecessary in women under 40 years old without historj of chest pain 1 coronary risk factors or aortic valve desease .
