RECONSTRUCTION OF INFECTIVE LOSS OF NOSE AND PERIORAL STRUCTURES DURING EARLY CHILDHOOD

Abstract

 Introduction & Objective: NOMA usually occurs in children living in unprivileged society, but rarely may it happen in higher socioeconomic areas of the world, to the immune compromised children because of severe gastroenteritis or measles infections. Normal mouth flora (Borrelia vincenti. Fusiformis fusiformis, staphylococci and anaerobes) are the main etiology for severe destruction of nose and perioral area.Materials & Methods: We present two cases who developed severe destructive lesion of nasal and perioral area after a short period of febrile debilitating illness. These two patients were referred to our center many years after developing the disease, for correction of sequelae and disfigurement.We present the modalities of NASAL RECONSTRUCTION for this devastating scar forming nasal mutilation and reviewing the literature for similar experiences.Results: NOMA is a destructive disease requiring a complex therapeutic strategy, different techniques and flaps that have been used in NOMA history. Skin over the nasal bridge as an inferior pedicle flap for nasal lining, costochondral segment of 11th rib for skeletal augmentation and forehead scalping flap for coverage, are feasible and easy to handle implement for total NASAL RECONSTRUCTION.Conclusions: A few items have to be taken into account, when planning to operate on a mutilated, scarred nose in a young individual. Another scar on the face should be avoided or donor site morbidity should be kept in minimum. Good Aesthetic result without proper nasal airway is less than acceptable and failure is always a possibility, so other solutions should be at hand.
