Abstract

 Introduction: Improving the practice & theoretical education of clinical residents needs new methods of education. Although medical students education has been studied & revised repeatedly, the clinical residents education hasn't received enough attention. More over, problem-based strategy and its variations, has presented new and efficient methods in medical education but if they are not applied correctly, students will not feel its benefits and advantages and even may prefer the traditional approach of education. Methods: Department of education of RCGLD provided a series of educational programs for internal medicine residents named as "Hard Talk on Gastroenterology & Liver Diseases" presented in the format of "case presentation & panel discussion". For each session, the proposed cases were chosen from the real patients considering program goals. Every single case was introduced and reported by a gastroenterology fellow. Each case presentation continued in a stepwise manner and stopped at every diagnostic and therapeutic decision point to let the discussion by participants and panelists to start. Meanwhile, he/she was asking questions to activate a bidirectional discussion between audience and panel group. Depending on the topic, some, or all of the invited specialists including gastroenterologists, pathologists, radiologists & surgeons were participating in discussion and the final result would be concluded by the fellow. In order to compare residents` attitude towards this method of education with classic education (lecture) a questionnaire was designed with 10 questions distributed among audiences after 6 educational sessions. The questions were about residents’ attitude of different educational aspects of new method including panel discussion, bidirectional discussion, expressing personal experiences and final conclusion and comparing them with lecture method. Results: Participants attitude was completely positive and all of the residents preferred the new method to the classic one. The possibility of bidirectional discussion and asking questions freely was mentioned by 95% of audience as the privileged point of new method. The same rate of residents believed that the "Hard talk" educational contents were more compatible with real patients in clinic, than classic lecture. More over, all of them were satisfied by the possibility of knowledge and experience transferring in a limited time. Interestingly, 50% preferred to hear just the scientific facts not the panel members personal experiences. And finally, 68% agreed with the presence of discussion and final conclusion. Conclusion: This study showed that an educational program for internal medicine residencies in the format of case presentation and panel discussion is a suitable and attractive method in residency education from the view point of learners.
