IDIOPATHIC NEPHROTIC SYNDROME: LINICOPATHOLOGICAL STUDY IN CHILDREN'S TEACHING HOSPITAL OF TABRIZ

Abstract

 Background & Aims: IDIOPATHIC NEPHROTIC SYNDROME (INS) is the most common glomerulopathy in children. There are variations in clinic pathological features, based on genetic and environmental factors.The aim of this study is to determine the clinical and pathological (in cases who needed renal biopsy) features of INS in children who referred to Children's Hospital of Tabriz and compare the results with literature.Materials & Methods: From September 1999 to March 2004, 77 patients with INS (out of 103 patients with NS) were studied from the point of demographic features, response pattern to medication, recurrence rate, histopathology and outcome. Indications that were considered for renal biopsy were: resistance to steroid, gross hematuria, hypertention, renal insufficiency, hpocomplementemia and age <1 or >10 years. Also patients with frequent relapse or steroid dependency underwent biopsy if they needed cytotoxic therapy.Results: 77 of 103 patients (74.7%) had INS, the mean age at onset of INS was 4.6±2.7 years old, male to female ratio was 2.1, response to standard prednisolon therapy was observed in 76.6% of INS, and 23.4% were resistant to prednisolon. The mean age at onset of nephritic syndrome was significantly higher in steroid resistant group than in the steroid responders. Also age of onset of more than 8 years was detected as a factor predicting poor response to steroid. In steroid responders recurrence rate and steroid dependency were 58.6% and 20.6% respectively. In 35 biopsied cases focal and segmental glomerulosclerosis and minimal disease change were the most common histopathological subtypes respectively.Conclusion: In this study INS comprises a lower percent of total nephritic syndrome in comparison with western countries, but its sex and age distribution and STEROID RESPONSE pattern is similar to them. Also higher age at onset of INS is a risk factor for steroid resistance.
