OUTCOME OF GUILLAIN-BARRE SYNDROME IN CHILDREN

Abstract

 Background: GUILLAIN-BARRE SYNDROME is the most common cause of ACUTE FLACCID PARALYSIS in CHILDREN. Recent advances in electro physiologic data and immunologic causes of the syndrome have led to development of a new reclassification of the disease as well as its treatment methods and OUTCOME. The aim of this study was to investigate the long-term OUTCOME of GUILLAIN-BARRE SYNDROME (GBS) in CHILDREN. Methods: During 10 years (1993-2003), 30 patients with GBS were admitted to pediatric clinics of Mashad University of Medical Sciences. Factors as age, sex, antecedent infection, cranial nerve and autonomic involvement, hospitalization duration and OUTCOME at 1.5 years after onset of disease were studied in these CHILDREN. Findings: Out of 30 patients, 14 (46.7%) were younger than 6 years and 16 (53.3%) were older. Fifteen patients (66.7%) had a history of respiratory illness, and 8 (26.7%) gastrointestinal diseases. Cranial nerves were involved in 46.7% and autonomic nerves in 26.7%. Fourteen (46.7%) patients had demyelinating type, 8 (26.7%) motor axonal and 8 (26.7%) motor-sensory axonal type. Eleven patients (36.7%) had a hospital stay of less than one week and 19 (63.3%) more than one week. A statistically significant correlation was observed between GBS type and the duration of in-hospital stay (p=0.028). Two-third of patients were followed for 1.5 years. Of these 15 (71%) achieved complete recovery while 6 (28.5%) developed neurological deficits. According to the above factors and long-term OUTCOME, autonomic involvement and motor-sensory axonal type were apparently associated with more neurological sequel (p=0.022 and p=0.004 respectively). No mortality was seen. Conclusions: 1.5 years follow up of patients, indicates that recovery in demyelinating form of this disease is expected, while axonal type leaves motor sensory sequel.
