Title

ETIOLOGY OF PERSISTENTLY ELEVATED AMINOTRANSFERASES LEVEL IN BLOOD DONORS FROM TEHRAN CENTER OF BLOOD TRANSFUSION

  90-94

Abstract

OBJECTIVES: Serum ALT level is a relatively sensitive and specific indicator of liver cell injury. Causes of elevated ALT levels vary depending on the population under study. The aim of this study was to determine the prevalence and causes of elevated ALT levels among Iranian healthy blood donors in Tehran. METHODS: A total of 1959 (1465 male, 494 female) randomly selected blood donors from Tehran were enrolled. After obtaining a history about alcohol and medication use, body mass Index (BMI), viral markers and ALT levels were measured. If ALT was elevated (>40 UIL), it was rechecked twice within 6 months. Blood donors with≥2 times elevated AL T levels (persistently elevated ALT) were enrolled for further evaluations such as liver sonography. RESULTS: A total of 100 donors (5.1%) had elevated ALT levels at the first measurement. Fifty- two (2.6%) had persistently elevated AL T levels. The causes of persistently elevated ALT were presumed NONALCOHOLIC FATTY LIVER DISEASE in 88.4%, hepatitis C in 7.7%, alcoholic and drug-induced hepatitis each in 1.92%.CONCLUSION: NONALCOHOLIC FATTY LIVER DISEASE is the most common cause of elevated ALT in the asymptomatic Iranian blood donors. NONALCOHOLIC FATTY LIVER DISEASE is emerging as the most common cause of hepatitis in Iran, similar to the developed countries. This may be due to changing life style of Iranians in the last decades which parallels the pandemic of obesity.
