ERADICATION RATE OF HELICOBACTER PYLORI USING LOW DOSE FURAZOLIDONE IN TRIPLE & QUADRUPLE THERAPIES VERSUS METRONIDAZOL IN TRIPLE THERAPY IN DU PATIENTS

Abstract

 INTRODUCTION: ERADICATION of H pyloriis essential for the effective treatment of PEPTIC ULCER disease. Triple therapy, is recommended as the first line treatment in ERADICATION of H. pylori. We aimed at comparing the efficacy of a quadruple & triple therapies containing low dose FURAZOLIDONE versus a triple therapy containing Metronidazole.METHOD: 150 patients with endosmotically confirmed duodenal ulcer and H.pylori infection were randomly enrolled to receive Omeprasol 20 mg BID, Amoxicillin 1000 mg BID and FURAZOLIDONE 100 mg BID (OAF) or Metronidasole 500 mg BID (OAM) or both FURAZOLIDONE 100 mg BID and Bismuth subcitrate 24 Omg BID (OABF) for 2 weeks H.pylori ERADICATION was assessed 12 weeks after the end of therapy with 14 - C urea breath test. RESULT: 150 patients (88 males and 62 females, mean age 40.3±11.5) were assigned. Male to female ratio, mean age, smoking state, history of gastrointestinal bleeding, NSAIDs use, mean duration of symptoms and greatest diameter of ulcers were not significantly different in three groups. 145 subjects completed the study. Excellent compliance was observed in 94% in OAF, 96% in OAM and 98% in OABF. Severe side effects were observed in 8% of OAF including malaise, anorexia and diarrhea and 4% in OAMI including headache and dry mouth and 6% in OABF including nausea, malaise and dizziness. Intention to treat analysis of the ERADICATION rates was 54% in OAF, 40% in OAM and 72% in OABF. The per protocol ERADICATION rates were 57.4% in OAF, 40.8% in OAM and 73.4%in OABF.CONCLUSION: triple therapies regimens are not recommended for H.pylori ERADICATION in Iranian patients with Du. Although the ERADICATION rate of Hp was significantly higher in the low dose FURAZOLIDONE containing quadruple regimens, but the ERADICATION rates were still less than the gold standard and there is still a need for further studies on quadruple therapies Containing a variant dose of FURAZOLIDONE or in other combination.
