Background & Aim: Medication errors are among the most common accidents in the nursing profession. The primary and natural consequence of such errors will be an increase in the hospitalization duration and an increase in costs, which in some cases can lead to severe injury and even death of the patient. This study aimed to determine the frequency and effective factors on reporting medication errors in nurses. Materials & Methods: This descriptive cross-sectional study was performed in Golestan province teaching hospitals. 340 nurses working in the hospitals were included in the study by random sampling method. Data were collected using demographic questionnaire as well as questionnaires on the frequency of medication errors and effective factors in reporting medication errors, and then analyzed using descriptive and inferential statistical methods by SPSS-23 software. Results: The frequency of medication errors in the nurses in the second half of the year 2021 was 87. 1%. formal reporting of medication errors was 7. 1% and informal reporting was 18. 2%. From the nurses’,view, among the factors affecting medication errors the highest scores were related to individual and mental condition errors, management process, patient and ward error, medication and physician errors, and finally nurses' professional errors. Conclusion: The three areas of factors related to the nurse, ward, and management should be considered by the managers of the respective hospitals in cooperation with the hospital matrons. If these factors are eliminated, the employees will report work errors accurately and will fully follow up their errors.

Abstract:

Background & Aim: Stroke is the second cause of disability and the third cause of death after heart diseases and cancers, which leads to major changes in lifestyle of the patients. The complications of this disease reduce self-care behaviors. Based on the evidence, implementing programs and interventions such as self-management training can be helpful. Therefore, the present study was conducted with the aim of determining the effects of education based on Encourage Autonomous Self-Enrichment (EASE) programs on medication compliance of stroke survivors in 2021. Materials & Methods: This study was an interventional study (quasi-experimental) with two groups (experimental and control) as pre-test and post-test that was performed on 55 stroke survivors in Mahabad city. Samples were selected by convenience Sampling method and randomly assigned into control (28) and experimental (27) groups. Data collection tools included patients' demographic information form and Moriski's medication compliance scale. The intervention included implementing education program by the EASE method for 12 days at the patients' homes and face-to-face in the presence of the patient's caregiver. Data analysis was done using SPSS version 22 statistical software and descriptive and inferential statistics. The statistical significance level was considered as less than 0. 05. Results: This study showed no statistically significant difference in medication adherence in patients of the control and experimental group before intervention (p>0. 05),however, a statistically significant difference was observed after the intervention (p<0. 05). Conclusion: Based on the findings of the present study, it was found that training by the EASE program is effective in these patients. Therefore, the authorities of the health and treatment system should adopt appropriate solutions for the EASE program of these patients and the training of their families and their active participation in the field of caring for these patients in order to improve the quality of life of these patients and avoid their re-hospitalizations and deaths as much as possible.

Abstract:

Background & Aim: Training transfer is necessary to improve the quality of health services provided by nurses. Despite substantial investment in healthcare training, studies show that only a small portion of training is transferring to the workplace. Therefore, the present study was designed and conducted with the aim of investigating training transfer among nurses using the theory of planned behavior. Materials & Methods: Correlational-predictive research design was used in this research. The target population consisted of all 78 nursing staff currently working at Imam Hossein hospital, Tehran, Iran, who participated in the documentation workshop. Using the method of Hair et al., 53 subjects were selected by random sampling method. In order to collect the data, a researcher-made questionnaire, the theory of planned behavior, and a reporting checklist were used. In order to analyze the data, structural equation modeling using Smart-PLS2 software was used. Results: The results showed, attitude toward training transfer subjective norms about training transfer, and behavioral control over training transfer significantly predicted training transfer intention. Also, the results showed that the perceived behavioral control towards the transfer of education and the intention to transfer education are significant predictors of nurses' education transfer behaviors. Conclusion: The theory of planned behavior can predict training transfer intention and training transfer behavior among nurses. Therefore, this theory can be a guide for healthcare officials to create a suitable environment for education transfer.
