EVALUATION OF RELATIONSHIP BETWEEN PREVALENCE AND INTENSITY OF DIABETIC RETINOPATHY AND NEPHROPATHY AMONG TYPE 2 DIABETIC PATIENTS REFERRED TO GOLESTAN HOSPITAL DIABETES CLINIC

Abstract

 Objective: To evaluate the prevalence, and the correlation as well as the intensity of diabetic RETINOPATHY and diabetic NEPHROPATHY among type 2 diabetic patients referring to Golestan hospital.Subjects and Methods: In this cross sectional study, type 2 diabetic patients were screened for diabetic NEPHROPATHY and diabetic RETINOPATHY by evaluation of 24h urine albumin and indirect ophthalmoscopy. One hundred and ten patients that had RETINOPATHY +/or NEPHROPATHY were include to study.Results: Among the selected 110 patients, 51 patients (51.8%) had microalbuminuria, 29 patients (26.3%) had macroalbuminuria and 24 patients (21.8%) without NEPHROPATHY. 62 patients (56.3%) had NPDR, 25 patients (22.7%) had PDR and 23 patients (20.9%) without RETINOPATHY. There was a significant correlation between the severity of RETINOPATHY in NPDR stage and the severity of NEPHROPATHY (p= 0.000). However, at PDR stage, there was no similar correlation (p>0.05). Prevalence of NEPHROPATHY in retinopathic patients was 72.4% (p<0.01). On the other hand, the prevalence of RETINOPATHY in patients with microalbuminuria was 66.6% and in patients with macroalbuminuria this figure was 86.2% (p<0.01).Conclusion:With progression of NEPHROPATHY from microalbuminuria to macroalbuminuria, RETINOPATHY progresses in patients at NPDR stage. However, during the PDR this possibility does not exist. The high correlation between the incidence of NEPHROPATHY and RETINOPATHY during NPDR stage in type 2 diabetic suggests that special attention to both the renal function and retinal integrity need to be given to these types of patients.
