IS THERE ANY RELATIONSHIP BETWEEN DIETARY DIVERSITY SCORE AND METABOLIC SYNDROME IN TEHRANIAN ADULTS?

Abstract

 Background: METABOLIC SYNDROME is a clustering of metabolic disorders including central fat accumulation, insulin resistance, abnormal glucose homeostasis, dyslipidemia and high blood pressure. Although diet has an important role in the development of METABOLIC SYNDROME, there is little emphasis on food variety in relation to reduction of chronic disease. This study was conducted to evaluate the relationship between DIETARY DIVERSITY SCORE (DDS) and METABOLIC SYNDROME in a group of Tehranian adults.Materials and Methods: In this population-based cross-sectional study, a representative sample of 581 subjects (295 male and 286 female) older than 18, residing in Tehran, were participated. Usual dietary intake was assessed using two 24 hour dietary recall questionnaire. Weight and height were measured according to standard protocols and body mass index was calculated. Fasting blood samples were taken for biochemical measurements and blood pressure was assessed according to standard methods. The main groups were divided in to 23 subgroups according to Haines et al (1999). Grain group categorized in to seven. Fruit was divided into 2 subgroups and vegetables into 7 subgroups.4 subgroups of meat and 3 subgroups for dairies were considered. To be counted as a "consumer" for any of the food group categories, a respondent needed to consume at least one-half serving as defined by the Food Pyramid quantity criteria, at any time during the 2-day survey period. It did not need to be eaten all at ones. Each of the 5 broad food categories receive a maximum diversity score of 2 out of the 10 possible score points. METABOLIC SYNDROME was defined according to ATPIII. Subjects were categorized based on quartile cut-points of DIETARY DIVERSITY SCORE.Results: Mean (±SD) of age and body mass index was 37±12year and 25.7±4.3kg/m2 respectively. Mean (±SD) of DIETARY DIVERSITY SCORE was 6.15±1.02. Higher DIETARY DIVERSITY SCORE was associated with lower fasting blood glucose (r=-0.21, p<0.05), and systolic blood pressure (r=-0.25, p<0.05). The probability of having METABOLIC SYNDROME, decreased with quartiles of DDS (odds ratios among quartiles: 1.33, 1.21, 1.17, 1.00, P<0.05). After controlling for confounders, a significantly decreasing trend was observed for the risk of having HYPERTENSION (odds ratios among quartiles: 1.43, 1.36, 1.21, 1.00, P<0.05) and IMPAIRED GLUCOSE HOMEOSTASIS (odds ratios among quartiles: 1.39, 1.21, 1.11, 1.00, P<0.05). Conclusion: DIETARY DIVERSITY SCORE has inverse association with METABOLIC SYNDROME and some of its features. Therefore, a varied diet can prevent from the metabolic disorder.
