Abstract

Introduction: We investigated the effect of oral clonidie PREMEDICATION before cataract surgery in patients underwent PHACOEMULSIFICATION with TOPICAL ANESTHESIA on PAIN severity, SEDATION, hemodynamics and patients and surgeon satisfaction.Methods: In a randomized double blind study, the effects of oral CLONIDINE PREMEDICATION as a sedative and analgesic agent were studied in 60 patients (40-80 years, ASA grade I, II and III) undergoing PHACOEMULSIFICATION under TOPICAL ANESTHESIA (tetracaine 0.5%) and systemic SEDATION (Fentanyl 1-1.5 a g/kgiv) The control group (n=30) received oral CLONIDINE 3 a g/kg 60 min before surgery and another group (n=30) received oral placebo. Then the PAIN severity (VAS) and SEDATION score (Ramsay), mean arterial pressure and pulse rate, patients and surgeon satisfaction and postoperative analgesic requirement were evaluated. Data was analyzed by X2 and t-test. Results: There was no significantly difference between two groups in order to demographic data, blood pressure and pulse rate and operation time. Postoperative PAIN score at 1, 2, 6 and 24 hours after. operation in CLONIDINE group was significantly lower than that of placebo group (P<0.05). There was no statistical difference in PAIN score, SEDATION score and surgeon satisfaction score during operation. Postoperative SEDATION score in CLONIDINE group was significantly better than placebo group (P<0.05). Patients satisfaction score and mean analgesic time were significantly greater in the CLONIDINE group than in the placebo group (P<0.05). Discussion: Oral CLONIDINE produces postoperative analgesia and SEDATION with good satisfaction for patients with cataract surgery under PHACOEMULSIFICATION in TOPICAL ANESTHESIA. There was no effect on intraoperative PAIN and SEDATION.
