Abstract

 The pressure in air-filled endotracheal cuffs increases steadily throughout general anesthesia with nitrous oxide (N20). High cuff pressures can be responsible for local ischemia, which may induce tracheal mucosal injury. The propose of this prospective randomized clinical trial was to compare the incidence of cough, sore throat, and hoarseness after using saline or air to inflate the endotracheal tube cuff. One of the variables that associated with these complications is intracuff pressure. 120 patients undergoing general anaesthesia with tracheal intubation <1h in duration were randomized into two groups. The endotracheal tube cuff was inflated to 'no air leak' with air in Group 1 (n= 60) and with saline in Group 2 (n=60). The patients were all ASA physical status I, adult patients undergoing general endotracheal anesthesia for transurethral lithotripsy (TUJ). Patients evaluated for these complications at 24 hours after intubations. The incidence of cough, sore throat, and hoarseness in Group 1 was 36.7,25, and 11.7 percent and in Group 2 was 30, 18.3, and 10 percent respectively; but there aren't any significant differences between them (P value> 0.05). IMPLICATIONS: In patients anaesthetized with nitrous oxide, the inflation of the tracheal tube cuff with saline doesn't have any significant advantage over inflation with air to reduce the incidence of cough, sore throat, and hoarseness after endotracheal intubation.
