Abstract: 

Background and Objective: Sexual problems in women are highly prevalent and frequently are associated with personal distress and impaired quality of life. The aim of the study was the validation of a translated and culturally adapted Iranian version of the FSFI -the IV-FSFI (Iranian Version of the Female Sexual Function Index).Subjects and Methods: In this cross-sectional study, 448 women (19 to 54 yr), after applying inclusion and exclusion criteria were eligible for this study. The instrument was translated into Persian, back-translated, and pilot tested to ascertain cultural sensitivity. Construct validity was evaluated by explanatory and confirmatory factor analysis and internal consistency was estimated with Cronbach’s alpha and the average inter-item correlation. In addition, the test-retest reliability of the measure was determined at a 4-week interval (n=362). Discriminant validity was assessed with between groups analysis of variance.Results: The overall test-retest reliability coefficients were high for each domain of the IV-FSFI (r ranging from 0.73 to 0.86) and the internal consistencies within the acceptable range (a from 0.72 to 0.90). Principal component analysis with varimax rotation revealed a best fitting five-factor structure similar to the original FSFI. CFA confirmed the underlying domain structure, supporting the factorial validity of the IV-FSFI.Conclusion: In conclusion, the newly developed IV-FSFI has demonstrated to be a reliable and valid instrument with good psychometric properties that allows a quick and accurate preliminary screening of women with unknown sexual health status in clinics and others medical settings.

Abstract: 

Background and Objective: Pulmonary tuberculosis (TB) is the most common form of the tuberculosis in HIV infected patients that can affect on progression of HIV infection .The aim of this study was to investigate the impact of HIV infection on the clinical presentation of tuberculosis.Subjects and Methods: We extracted the information of admitted patients in infectious ward of Razi hospital with pulmonary tuberculosis diagnosis from 2004-2009. Outcome of this stage was 204 patients. Then we divided these patients in two groups, group 1 patients with HIV (34 cases) and group 2 without HIV infection (170 cases). Finally, we compared the clinical manifestations between these two groups. Data analysis with Chi square test and SPSS software was done.Results: Tuberculosis was more common in men in both groups (p<0.001). The mean age of HIV positive patients was lower than HIV negative patients. (31.85±8.52 vs. 44.33±10.34), (p=0.001). Fever, weakness and hemoptysis were more frequent in HIV positive patients (%94.11 vs. %81.17, %58.82 vs. %50.58 and %20.58 vs. %7.05), (p=0.07, p=0.23, p=0.01). Whereas chronic cough, sputum, weight loss and night sweating were more prominent in HIV negative patients .(%95.29 vs. %73.52, %87.64 vs. %70.58, %71.17 vs. %38.23 and %61.76 vs. %41.17), (p= 0.03, p= 0.04, p=0.009, p=0.04).Conclusion: According to the study in these two groups it is important to consider tuberculosis in differential diagnosis of every HIV infected patient not only with cough and sputum but also with fever and constitutional symptoms.

Abstract: 

Background and Objective: Discovery of leptin has led to numerous experiments for better understand of its function, and many of these studies have focused on leptin’s response to both acute exercise and exercise training. The aim of this research was the investigation of acute effects of an aerobic exhaustive incremental exercise session on serum leptin and plasma lipids in trained men 19-26 years old.Subjects and Methods: Voluntary twenty four trained divided into trained (12 subjects) and control (12 subjects) groups, randomly. Exercise session includes Astrand exhaustive incremental test on treadmill which have been done between 8-10 am. Blood sampling gathered pretest, post test and 9 hours after training. For data analysis we have employed two way repeated measures ANOVA, paired T-Test and Pearson correlation coefficient using SPSS software, version 15.Results: Results demonstrated serum leptin immediately increase after training in control group but this change statistically was not significant (p>0.05). Plasma LDL-c and total cholesterol significantly increase immediately after training in pretest values (p>0.033). Significantly negative correlation was found between serum leptin immediately after test and pretest plasma LDL-c in trained group (r=-0.65, p>0.022). In general, there are not any significant differences between two control and trained groups in any measured blood factors during three bouts measurement.Conclusion: An aerobic exhaustive incremental exercise session statistically has not acute effect on serum leptin in trained men 19-26 years old.

Abstract: 

Background and Objective: Duchenne muscular dystrophy is an X-linked genetic disorder resulting from mutation or deletion in the Dystrophin gene. The aim of this study was to evaluate the primary diagnosis of affected individuals that have been referred to the genetic lab of the Shafa hospital in Ahvaz. Progressive muscle weakness was present in all the patients.Subjects and Methods: DNA from peripheral blood was extracted from affected patients and subsequent multiplex-PCR was performed to determine putative deletions in the Dystrophin gene.Results: in 53% of cases were deletions identified in exons 44-51 in the Dystrophin gene and therefore the clinical diagnosis could be confirmed. On the other hand, we found no deletion in 47% of cases.Conclusion: it seems that the patients suffering Duchenne muscular Dystrophy in Ahvaz show, independent to their ethnicity, the gene inactivating deletion in the end part of the Dystrophin gene. These results would be used for the differential and for the prenatal diagnosis in the Khuzestan province.

Abstract: 

Background and Objective: The high prevalence of Unilateral Chronic Ankle Instability (UCAI) in human society and particularly in athletes, and possibility of its related side effects to become chronic led to decrease their ability to fulfill the expected roles in different aspects of life. So, in order to return them back to sport activities, a reliable functional test is needed. Hence, the purpose of this research program was decided to study the effect of Star Excursion Balance Training (SEBT) on the distance reached by the uninvolved leg while standing on the involved leg, and its effect on dynamic balance of subjects with UCAI via agility hop test.Subjects and methods: Twenty four patients with UCAI, were randomly assigned to the study and were divided into two groups of 12 subjects. The control and test group participants underwent the conventional and modern physical therapy interventions respectively.Results: Comparison of results regarding the SEBT scores of the test group is witness of improvement of dynamic balance in these patient. These results suggest that despite the improvement that was shown with respect to increase in the reached distance by the unaffected leg, the significant difference between two treatment methods, was evident only in 4 out of 8 directions of anterior, anteromedial, posterior and posteromedial (P£0.05 respectively). Descriptive and analytic (independent t- test) was carried out by SPSS 15 software.Conclusion: Result of this present study revealed the fact that both PT intervention programs were effective in improvement of the dynamic balance of patients with UCAI. However, a significant difference between two intervention programs regarding the reached distance is evident from the session 10 to 12 in 4 directions. So, in order to consider a significant difference between these two intervention programs, it might be essential to increase the number of treatment sessions to more than 12.

Abstract: 

Background and Objective: Pertussis is a highly communicable respiratory infection which can cause chronic cough. Pertussis can attack any age. Despite childhood vaccination, adults or haven’t immunity or have few immunity. The aim of this study was determination of frequency of pertussis among adults with prolonged cough.Subjects and Methods: This descriptive cross sectional study was performed on adults who had coughed for more than 2 weeks and there were no reasons for prolonged cough. Diagnosis of B.pertussis was performed by culture on Bordet-Gengou medium, which gets by deep throat soap sample. The results were analyzed by SPSS with t and chi square tests.Results: One hundred twenty three patients [57 (46.3%) men and 66 (53.7%) women] with the mean age of 41.4±17.7 years were enrolled to study. The mean duration of cough was 22.6±14.6 days. Ninety five (77.2%) had a history of vaccination. Culture of deep throat soap sample was positive among 23 (18.7%) patients [15 (12.2%) men and 8 (6.5%) women] who arrived in the study.Conclusion: In this study, B.pertussis confirmed in 23 adults (18.7%) by culture of deep throat soap sample. It’s suggested that physician in the visit of adult patients with prolonged cough more than 2 weeks; also pay attention to pertussis as well as this group becomes a potential source of pertussis for infant, vast adult vaccination with acellular pertussis vaccine is noteworthy for public health organization.

Abstract: 

Background and Objective: Snake venom particulary those belonging to Crotalidae and viperidae families, are known to contain number of components affecting blood coagulation.The Aim of this paper is to describe the isolation a specific blood coagulation factor X activator from the crude venom of Iranian Vipera lebetina.Material and Methods: Factor X activator was purified from two hundred mg of crude venom by gel filtration on sephadex G-100 and two step ion-exchange chromatography on DEAE-cellulose (DEAE-52).Results: It showed a single band in sodium dodecyl sulfate-polyacrylamid gel electrophoresis (SDS-PAGE) in non-reducing condition. The mol. wt was estimated to be 78000Da by SDS-PAGE the activator, activated factor X to Xa in presence of calcium ions. It could not activate prothrombin, no had effect an fibrinogen and thus appeared to act specifically on factor X.The activator no amidolytic activtiy on factor Xa substrate benzoyl-Ile-glu-Gly-Arg-p-nitracnilide (S-2222).It had no arginine esterase activity toward substrate benzoyl-agrinine ethylester (BAEE) while crude venom hydrolyzes this substrate.Conclusion: The results of this study showed that The activator do act specifically on factor X.

Abstract: 

Background and Objective: Todays, The etiology of binge eating has not been definitively determined. Treatment must address the somatic, the behavioral, and the psychological problems. The aim of this study was to determine the effectiveness of cognitive behavior group therapy on reducing symptoms of binge eating disorder in woman of Ahvaz overeating anonymous.Subjects and Methods: Subjects were consisted of 30 patients with binge eating disorder that were selected from overeating anonymous. Sampling was convenience then patients who were diagnosed with binge scale and clinical interview, were randomly assigned to experimental and control groups. The experimental group received 6 weekly sessions of cognitive behavior group therapy that the control group did not received. It was hypothesized that cognitive behavior group therapy decrease the symptoms of binge eating disorder of the experimental group as compared with the control group. The binge scale was administered to both groups before and after treatment. SPSS 16 software was used for data analysis.Results: The results of covariance analysis showed that cognitive behavior group therapy significantly reduced the symptoms of binge eating (F=107.16, p<0.001) in the experimental group as compared with the control group.Conclusion: This study showed that cognitive behavior therapy is effective on reducing symptoms of binge eating, and this method can be one of the treatment choices for this disorder considered by the therapist.

Abstract: 

Background and Objective: Intestinal parasitic infections are one of the health problems in developing countries. Now one to two milliard people in the world are affected by ascariasis annually, and every year people are spending heavily to treat. All the problems of the ascariasis emission can be controlled by health education programs, which cost less than treatment of the disease. This study was performed to assess health knowledge, attitude and practice of mothers referred to health centers of Ahvaz about ascariasis of children in 2006.Subjects and Methods: In This descriptive analytical research 214 mothers were selected randomly from ten health centers. Using questionnaires the data were collected. Gathered data were analyzed by SPSS-13 software.Results: The results implied that 77.6% of the mothers were housewives. 9.8% were illiterate. The findings indicated that the knowledge of the mothers was (77.6%) at a weak level, their attitude was (68.2%) at the middle level, and their practice was (84.1%) at the weak level about ascariasis control and prevention.Conclusion: In this study there were a significant correlation between health knowledge, attitude and practice at confidence interval of 95%. A significant relation was also seen between health knowledge, attitude, practice and mothers occupation and literacy at confidence interval of 95%. With regard to the results of this study the government should pay more attention to improve efficient and effective health education programs to increase the knowledge, attitude and practice about prevention of the ascariasis disease.

Abstract: 

The management of children with major abdominal injuries has changed significantly over the past 2 decades. Few surgeons have extensive experience with massive abdominal solid organ injury requiring immediate surgery. Prompt identification of potentially life-threatening intraabdominal injuries with rapid resuscitation and therapeutic intervention is now possible in the overwhelming majority of children. CT has become the standard of care in the evaluation of the pediatric trauma patient. Modern generation CT scanners are highly sensitive in the evaluation of possible solid organ and retroperitoneal injuries. There must be a high index of suspicion for a hollow viscus injury in the child with free intraperitoneal fluid and no identifiable solid organ injury on CT. Clinician-performed sonography for the early evaluation of the injured child has been shown to be useful in many situations but does have limitations. A negative FAST exam does not exclude a significant solid organ or hollow viscus injury. The ready availability of high-resolution CT scanners and the nonoperative management of many pediatric injury patterns have increased, thus minimizing the utility of DPL. The use of laparoscopy for the injured child may have its place in the evaluation armamentarium of the hemodynamically stable patient. The spleen and liver are the organs most commonly injured in blunt abdominal trauma, each accounting for one third of the injuries. Each nonoperative treatment of isolated splenic and hepatic injuries in stable children is now standard practice. The clinical experience accumulated over the past 20 to 30 years, which has settled these concerns, is reviewed.

Abstract: 

Small or large, single or multiple common bile duct (CBD) stone occur in 6-12% of patients suffering from gallstone. Incidence of CBD stone increases with age. 20-25% of patients who suffer of gallstone associated with CBD stone are above 60 years old. In westerns, CBD stones originate from gallbladder that pass from cystic duct to CBD, these named secondary stones. Primary stones construct in intra or extra hepatic biliary ducts.We present such a rare case, 75 years old lady with an unusual casting large CBD stone with 40mm diameter and 150mm long which extends toward hepatic branches proximally. We named it biliary staghorn due to shape similarity to renal staghorns. Patient had colici abdominal pain in right upper quaderant since several years ago. She have any complications as collangitis or pancreatitis. Diagnosis made by ultrasound and patient operated on for retrieval of stone by choledocotomy. Choledojejunostomy Reux-en-Y for biliary drainage.

Abstract: 

According to several reports the incidence of lingual thyroid in childhood are more than adults. This ectopic thyroid gland induced mild hypothyroidism with spectrum as normal or below normal.Generally, most cases of congenital ectopic thyroid appears as Cretinism.In this report we will introduce an adult with a lingual thyroid that induced adult hypothyroidism.
