Introduction: It is very important for internal quality control of medical centers to evaluate the diagnostic accuracy of frozen sections, because this diagnostic method forms the basis of patient treatment. This study aimed to determine the accuracy of frozen section in the diagnosis of breast lump.Materials and Methods: In Be’sat and Imam Reza hospital lab more than 25 cases per year are received for frozen section. Totally, 59 consecutive frozen sections of breast lumps were examined from 2008-2013 and were considered for analysis. Frozen section diagnosis given in breast biopsies performed in 5 years was evaluated against the final pathology report of permanent sections. Results: Age of patients ranged between 23 and 80 years (mean age 50.4 years). In 56 cases (98.24%), there was no difference between the frozen section diagnosis and final diagnosis. One case was false negative (1.69%). No false positive diagnosis was noted. In two cases (3.4%) frozen section diagnosis were deferred to permanent sections. The diagnostic accuracy of frozen section was 98.24%. The sensitivity, specificity, positive predictive value and negative predictive value were 97.61%, 100%, 100%, and 93.75%, respectively. Conclusion: High accuracy of the frozen-section diagnosis in the breast lumps was confirmed in our study.

Introduction: Transmission of Malaria has shown substantial heterogeneity. Evidently, considering all aspects of the transmission chain of malaria including vector, host, and malaria parasite are necessary for controlling malaria. The aim of this study is to examine the factors influencing on malaria transmission pattern in Chabahar city.Materials and Methods: In this case - control study, we included 128 newly diagnosed patients with malaria as case and same number of control from Chabahar, southeastern district of Iran, from March to September 2012. The case and control groups were matched individually by age and sex. Data collection is conducted based on form No. 10 which is related to the epidemiological situation. For data analysis, we used Conditional logistic regression. Results: The most common symptoms of malaria were headache and fever. Moreover, 35.1% of malaria cases were imported from neighboring countries. The use of mosquito nets, spraying, air conditioners, previous history of malaria, and education level were found to be protective factors and history of travel during last year was considered as the major risk factor for the disease. Conclusion: The most important and effective measure to control malaria is the incorporated management of vectors, that is using the multiple preventive approaches.

Abstract:

Introduction: Occupational stress has association with workplace exhaustion and faint emotional regulation. Also, clinical research has emphasized on association of pirituality and healthy. Studies have survived the role of spirituality and emotional regulation in psychological healthiness, but haven’t found research survived this variable in nurse’s stress.Method: This is a descriptive cross-sectional research performed on 170 nurses in a military hospital in Tehran who were selected by random multi-stage method. Data collecting tools were the Osipows job stress inventory, difficulties of regulations scales and Spirituality questionnaire. Data were analyzed by SPSS-19 software using Spearman correlation and Logistic Regression analysis tests. Results: the result from analyses is revealed that spirituality has significantly relation with occupational stress and emotional regulation, but emotional regulation hasn’t significant relation with occupational stress. Conclusion: occupational stress in military nurses can decrease with increase of spirituality.

The Goldman standard questionnaire was used for organizational justice and WHO standard questionnaire was used to meet and data were analyzed by SPSS 16 software. Results: There was a significant relationship between organizational justice and responsibility dimensions (p<0.05). Among the dimensions of responsibility, respectively the confidentiality of information, Dignity and basic amenities was more important, and among organizational justice dimension, interactive dimensions earned a higher rating. Conclusion: Considering that the distribution of salaries and bonuses to employees that has a positive effect on the staff of the health system responsibility and obtained score in this study has a lower average, therefore that officials and administrators of Military hospitals pay special attention to this dimension of organizational justice.

Introduction: Defense organizations increasingly interested to achieving of the occupational health and safety performance through controlling the risks goals and safety policy. This study aimed to effects of the establishment of occupational health and safety management systems on the environmental factors and employees satisfaction.Materials and Methods: This study was quasi-experimental and type of before and after approach. This study was performed in the MILAD industry complex in year of 2012. Data collection used by data collection form and the structured questionnaire and validity and its reliability were confirmed (a=%81).The study population using Cochran's formula included 100 of employees which were formed by random sampling. Results: The results showed that the mean level of sound intensity of 54.79 dB 51.72 dB were reduced, intensity stations in the limit of (8.30% to 4.70%)) increased, chemical agents from 2.14 to 4.11 mg/m3 is reduced, mainly gaseous CO (from 143.0 to 062.0) input CO (from 28.9 to 49.7).Conclusion: The effect of these systems on heterogeneous, there are some environmental factors so that the positive effect of sound intensity at four plants in one of the factories had no effect.

Abstract:

Introduction: Hepatitis A disease is caused by hepatitis A virus and transmitted by oral-fecal route and cause permanent immunity. It has relatively high prevalence in developing countries as the majority of people affected by the disease in childhood; therefore As a result, today the disease serology in adults has decreased. This study was conducted for evaluating HAV IgG seropositivity in adult personnel working in a military hospital.Material and method: In this descriptive- cross sectional study 150 healthcare worker over 20 years of a military hospital were evaluated for history of HAV infection in 2011. 5 mili-liter of serum sample of these patients has been sent to the laboratory for detecting HAV IgG by ELISA method. Data were analyzed by SPSS-15 and Pearson test. Results: Total seropositivity of HAV IgG was 17.3% and the most frequent age group was 20-29 years. There was significant difference in the frequency of Anti HAV IgG in the age groups (P<0.001), but not in gender (P=0.526).Conclusion: These findings indicate that HAV seropositivity in older age groups has decreased. Regard to HAV endemicity in our country and the risk of catastrophic pandemic, vaccination against it seems to be necessary.

Abstract:

Introduction: The aim of this article is review and analysis of the Meta cognitive therapy (MCT) as an effective intervention in the treatment of posttraumatic stress disorder (PTSD) in the past two decades.Materials and Methods: This study by using systematic review of studies to explore the underlying assumptions and interventions of the Meta cognitive therapy of post traumatic stress disorder and studies on the effectiveness of this approach are explored.Result: In MCT, negative Meta cognition beliefs, dysfunctional attention and cognitive processes (rumination, Meta worry, avoidance, thought suppression, continuous monitoring of threats) are considered as the main cause of PTSD. Unlike the cognitive behavioral therapy, treatment in MCT is not involved exposure and discusses the cognitive content, Instead, treatment and education focusing on the type of the thinking process. Conclusion: Meta cognitive therapy is an emerging approach that the result of systematic modeling and has led to various techniques that have been shown effective in scientific studies. This approach to the understanding and treatment of mental disorders, particularly PTSD has been very effective.
