Abstract

Purpose: To report a case presenting with DACRYOADENITIS as the earliest manifestation of systemic Wegeners granulomatosis (WG). Patient and findings: We examined a 41 year old woman who initially presented the symptoms of bilateral DACRYOADENITIS. Upper and lower respiratory tract involvement, scleritis and keratitis evolved subsequently. Cytoplasmic antineutrophil cytroplasmic antibody titer was found positive. The lacrimal gland and lung biopsy were consistent with WG. The patient responded well to treatment with cyclophosphamide and prednisolone.Conclusion: DACRYOADENITIS may be the earliest presenting manifestation of WG and by early administration of immunosuppressive chemotherapy, the disease may be controlled thus preventing the limited disease from progressing to a complete from and decreasing its morbidity and mortality.
