Background & Aim: Despite the prevalence of delirium after the coronary artery bypass graft surgery, inadequate studies were conducted on the prevention of delirium after this surgery. This study was undertaken to evaluate the effects of environmental simulation on delirium incidence after coronary artery bypass graft surgery. Materials & Methods: A clinical trial was done in the cardiac ICU of Shahid Beheshti Hospital, Kashan, Iran. A sample of 100 patients who were candidate for coronary artery bypass graft surgery were randomly allocated to a control and an intervention group. For patients in the intervention group, postoperative environmental simulation was used to simulate their household environments, while patients in the control group were cared in usual postoperative environment. The Mini-Mental State Examination test was used to assess delirium three hours after regaining postoperative consciousness and then, twice a day until the third postoperative day. Data analysis was performed via the independentsample t, the Chi-square, the Fisher’ s exact, and the Mann-Whitney U tests. Results: The number of patients with delirium or at risk for it in the intervention group was lower than the control group at different measurement time points. Although the between group differences were not statistically significant. Conclusion: The number of patients with or at risk of delirium was less in the intervention group. This finding is clinically important. However, as the results were not statistically significant, further investigations are needed in this area.

Abstract:

Background & Aims: Organizations always struggled to reach the highest levels of employee performance in line with their goals. Hospitals are no exception to this rule too, especially nurses who are the most important occupational groups in the hospital. The purpose of this study was to evaluate the dimensions of the internal environment and the five personality traits in nurses' job performance in an educational hospital. Materials & Methods: Nurses working in a teaching hospital proportional to different wards were selected and 89 person entered the research. The three internal organizational questionnaires (Weisbord), Five Personality traits (NEO) and Job Performance Evaluation (Paterson) were data collection tools. 185 nurses entered the study as samples by using appropriate allocation in each hospital ward. Data were analyzed by SPSS20 software using descriptive statistics and artificial neural networks. Result: In Personality trait questionnaire, the dimensions of accountability (44. 13 ± 5. 60) and neuroticism (32. 86 ± 4. 93) were the most and the lowest mean scores. In the internal organization questionnaire, the dimensions of rewards (3. 81 ± 1. 11) and communication (4. 74 ± 1. 14) were respectively the best and worst of the studied dimensions. Comparison of importance coefficients in neural networks showed that the dimensions of purpose (from the interior of the organization, 100%) and type of employment (from demographic characteristics, 7. 20%) had the highest and the least importance in nursing job performance. Conclusion: Since in this research, purpose (from the interior of the organization) has the highest significance factor in nurses' performance, it is suggested that formulating clear goals and this group should interfere in organizational goals in order to achieve the highest level of job performance.

Abstract:

Paederus is a genus of small beetles of the family Staphylinidae (rove beetles), widely distributed around Northern Iran and known as Dracula in this region. It releases a potent vesicant and notorious toxin called pederin which results in mitosis blockage by inhibiting protein and DNA synthesis and causes a self-limiting form of acute irritant contact dermatitis known as Paederus dermatitis. Since there isn't any specific antidote to neutralize the toxin and its effects, the treatment is largely conservative, focusing on relieving symptoms. It seems the best method to prevent these complications is to avoid contact with the beetles in the first place and in case of contact, the affected area should be immediately washed with soap and water and/or alcohol-based rub-in cleanser to remove the irritant. Unfortunately, the lack of proper notification by healthcare officials and providers to the local inhabitants and the coincidence of high rates of traveling to this region with the peak season of Paederus activity leads to a larger population to be exposed to this insect and subsequently a high incidence of the disease. On the other hand, there are no precise statistics available for the incidence rate of Paederus dermatitis, characteristics of affected patients, and distribution patterns of insects due to a lack of a comprehensive system for recording and reporting the cases of this disease.

Abstract:

Background & Aim: Cardiovascular diseases are the most common cause of death in the world. Despite numerous pharmacological treatments, coronary artery bypass grafting surgery is still known as the first and best treatment choice for these patients. Fear and anxiety are the most prevalent psychological complications of patients undergoing coronary artery bypass grafting surgery. It seems that social support is a factor affecting these two variables. Hence, the present study was conducted to assess the correlation of perceived social support with preoperative fear and anxiety among the patients undergoing elective CABG in 2015. Materials & Methods: Among the elective patients undergoing coronary artery bypass grafting in the special departments of Imam Ali Hospital in Kermanshah, 100 patients (age: 55. 80 ± 2. 9 yr) were selected as subjects. Data were collected using questionnaires of individual characteristics, perceived social support of Sherborne-Stewart, coronary artery bypass graft scales (BGFS) and Spielbergers. The Spearman correlation test was used to examine the relationship between variables at the significance level of 0. 05. Results: Social support outcomes (57. 78 ± 10. 4), pre-operative anxiety (51. 11 ± 5. 8) and fear (57. 77 ± 7. 7) were shown. There was an inverse and significant relationship between perceived social support and anxiety level (r =-0. 3). There was also a significant and inverse relationship between social support perceived variables and fear (r =-0. 2). Conclusion: The results of this study indicate the importance of perceived social support for decreasing the amount of fear and anxiety in these patients. Therefore, identifying the patient's support resources and promoting it, is necessary. Supportive nursing care can improve mental health of patients.
