Abstract: 

Background: Hospital surfaces and Staff hands are most common sources to transmission and spread Bacteria from hospital. b-lactame antibiotics are very important agents to control nosocomial infections. The spread of b-lactamase bacteria can disrupt the treatment process. This study was done to evaluate the frequency of b-lactamase bacteria isolated from staff hands and hospital surfaces.Methods: This study was a laboratory study and was performed in the years 2005 to 2007 in Alzahra (SA) hospital in Isfahan.274 samples were selected randomly from stuff hands and hospital surfaces. Hospital surface samples were collected with swab and NB (Nutrient Broth), from high and low contact surface and staff hand samples were collected with Finger Print method. Bacterial identification was performed with microbiological methods and b–lactamase production survey was performed with Acidometric method.Findings: Staphylococcus species were seen in 55% of 194 isolated Bacteria from hospital surface. Other bacteria that isolated from hospital surface were Bacillus species in 26.29%, Enterobacteriaceae in 9.80%, Pseudomonas species in 3.9%, other gram negative Bacteria in 4.51%, Streptococcus species in 0.5% of samples.60% of 80 isolated Bacteria from staff hand were Bacillus species. Staphylococcus species in 36.25% and Enterobacteriaceae in 3.75% were seen in stuff hands samples. According to b–lactamase test result, respectively 75.43% and 71.23% of isolate Bacteria from hospital surfaces and staff hands, produced b-lactamaseConclusion: The results of this study have shown the high frequency of b–lactamase enzyme in isolated Bacteria from staff hands and hospital surfaces. Improving quality of hand washing and disinfectant agent are very important methods to control nosocomial infections.

Abstract: 

Background: Cardiac rehabilitation and secondary prevention is an integral part of the treatment of patient with cardiovascular disease to date, however the effect of this programs on systolic and diastolic function is controversial. The aim of this study was to evaluate the effect of 8 weeks cardiac rehabilitation program on diastolic function and functional capacity in patients with myocardial infarction.Methods: twenty-nine post myocardial patient enrolled in this study, which was completed in cardiac rehabilitation group (15 men) and control group (14 men). Echocardiographic measures of diastolic filling and functional capacity by symptom limited exercise test based on naughton protocol on treadmill were performed at baseline and after 8 weeks of training. Data were analyzed using repeated analysis of covariance (ANCOVA) in P<0.05 level.Findings: After 8 weeks cardiac rehabilitation program, diastolic filling indicators did not change significantly (E velocity, A velocity, E/A ratio and deceleration time). But functional capacity increase significantly in cardiac rehabilitation group. But, functional capacity, resting HR and maximal HR improved significantly in the training group compared to the control group.Conclusion: This study revealed that Cardiac rehabilitation program for 8 weeks significantly improves functional capacity in post myocardial patients but diastolic function did not change significantly. It seems likely that the improvement in functional capacity largely pertain to non cardiac effects.

Abstract: 

Background: The population growth is an important issue affecting all dimensions of human life. Men play an important role in family planning. The present study aims to investigate men cooperation in family planning after a group training program of their spouses.Methods: This study was a quasi-experimental research. In this study the effect of a group training on contraception on 53 women who used natural method of contraception was measured on their husbands. Before and after intervention an awareness and practice questionnaire about contraception methods was completed in studied women and their husbands. Data analysis was done by SPSS software and descriptive and analytic statistical tests.Findings: A significant difference was found between the awareness scores of the intervention group and their spouses before and after the group training (P<0.001).51 percent of the studied women and their husbands underwent an effective contraception method after group trainingsConclusion: The results of the study indicated that women group training can increase men cooperation in family planning programs.

Abstract: 

Background: Nausea and vomiting of pregnancy (NVP) are seen in 50-90% of pregnancies. Mild to moderate NVP in the first trimester of pregnancy in most patients can be cured by ensuring them. However, in severe cases medical therapy to reduce nausea and vomiting is inevitable. Ondansetron as a drug effective in reducing nausea and vomiting is one of the therapies for the treatment of NVP that recently has been proposed. This study compared the effectiveness of Ondansetron and Metoclopramide in the treatment of NVP.Methods: In a clinical trial study 70 pregnant women with NVP were selected in Sahid Beheshti hospital in 2010 and randomly divided in 2 groups. The first group administered oral administration of Metoclopramide and the second group were treated by Ondansetron for 3 weeks. The daily frequency of nausea and vomiting and medication side effects were studied before starting the intervention and three days, a week, two weeks and three weeks after treatment in the two groups. Data analysis was done by X2, Fisher exact test and Student t–test.Findings: The results showed that there was not a significant difference between groups on the trend of change of nausea during treatment (P=0.27) but the number of vomiting in Ondansetron group was significantly lower than other group (P=0.04).Conclusion: Ondansetron have more favorable effect in controlling NVP. If the patient accept and this drug is safe in pregnancy, it can be used to control NVP during pregnancy.

Abstract: 

Background: The diagnosis of hyperparathyroidism is usually first suspected because of the finding of an elevated serum calcium concentration. The clinical presentation may be subtle and hypercalcemia may be mild. Single adenomas account for most cases of primary hyperparathyroidism. One of the atypical presentations includes normocalcemic primary hyperparathyroidism. Thyroid nodules come to clinical attention when noted by the patient, or as an incidental finding during the routine physical examination, or during a para clinical procedure. Ultrasound is also recommended for all patients to confirm the presence of nodularity. Fineneedle aspiration (FNA) biopsy is the most accurate method for evaluating thyroid nodules. Thyroid scintigraphy is used to determine the functional status of a nodule.Case report: In this study, we report a case of huge parathyroid adenoma and thyroid adenoma in the same time in a patient with nonspecific generalized body pain. Primary clinical and para clinical findings did not have any correlation with each other and could not guide us to the correct diagnosis.
